What to Expect during Gynecomastia Surgery (Male Breast Reduction)–
Gynecomastia surgery, also known as male breast reduction, is a surgical procedure that addresses and corrects enlarged or overdeveloped breasts in men. While liposuction or CoolSculpting can be effective in removing excess fat from men’s chest, the actual glandular breast tissue that develops in gynecomastia can only be removed by surgery.
The vast majority of gynecomastia cases involves the surgical removal of glandular tissue and/or skin that liposuction alone cannot resolve. In cases where the patient’s skin has stretched or sagged, a surgical approach is necessary to achieve optimal results.
In this article Waterford, MI area board certified plastic surgeon Doctor Ali explains what to expect during gynecomastia (male breast reduction) surgery.
Preparing for Gynecomastia Surgery
Your successful surgery actually begins several days in advance of the actual procedure. Dr. Ali’s office will give you complete and detailed instructions regarding your gynecomastia surgery, letting you know how to prepare in advance and what to expect following the surgery. In order to ensure that the procedure goes as smoothly as possible, it is very important to follow these instructions to the letter.
Of course, every case is unique. However, in general, you need to stop taking medications that would tend to thin your blood, such as aspirin, Advil, and even Vitamin E and many herbal products.
You should plan to stop eating and drinking at least 8 hours before your procedure.
You will want to wear comfortable clothing that is not tight or restrictive. And be sure to arrange for someone to drive you to and from your surgery. It is also advisable to have someone stay with you at least a night or two, as you will need a lot of rest and may be in some discomfort.
Gynecomastia Surgery Anesthetic
In most cases, gynecomastia surgery is considered an outpatient procedure, and is performed using either a general anesthesia or local anesthesia with sedation.
Waterford area board certified plastic surgeon Dr. Ali will discuss your anesthesia options and preferences at your consultation. Of course, any anesthesia will be administered by an experienced, qualified anesthesiologist.
Gynecomastia Surgery Incision
Generally, incisions are made either around the borders of the areola (peri-areolar incision) or following the natural creases of the breast. Dr. Ali is a trained, qualified cosmetic surgeon and takes great care in the placement of the incisions to be sure that any scarring is minimal and as inconspicuous as possible. Whatever scars occur will diminish with time, and chest hair usually camouflages any scars that remain. Even so, most men prefer whatever small scarring they may experience to having oversized breasts.
Gynecomastia Surgery
During the surgery, Dr. Ali makes an incision and removes the excess glandular tissue. While the glandular tissue must be cut out (excised), Dr. Ali may also use liposuction techniques in addition to excising (cutting out) to remove any fatty tissue. Prior to the surgery, Dr. Ali will select one of the various liposuction techniques that he believes will be most effective for your unique case.
Depending on the extent of your surgery, the areola may also need to be reduced, and perhaps the nipple will be moved to provide a more natural male contour.
Finally, if there is excess saggy skin that needs to be addressed, Dr. Ali will remove and tighten it.
After the surgery, Dr. Ali will apply dressings or bandages to cover the incisions and an elastic bandage or compression vest will support the surgical area and minimize swelling as it heals after surgery.
In some cases that involve the removal of a large amount of breast tissue, a small drain may be inserted under the skin so that any blood or fluid can drain. In most cases, however, these are not necessary. If your case does necessitate the insertion of a drain, Dr. Ali’s office will give you specific instructions outlining how to care for the surgical site and drains.
Recovery after Gynecomastia Surgery
Because you will be given an anesthesia, you must arrange in advance for someone to drive you home. Additionally, when you get home you will want to rest in bed for at least a day or two, so it is helpful to have someone stay with you to care for you.
Most patients are able to return to work about a week after surgery, and a gradual return to exercise is usually permitted after the few weeks. Of course, Dr. Ali will need to clear you before you resume heavy lifting or strenuous activities or exercise.
Gynecomastia surgery can be life-changing to men who have been uncomfortable with the appearance of an enlarged chest. Waterford area board certified plastic surgeon Dr. Ali has helped hundreds of his patients to achieve the results they desire. Men’s Plastic Surgery – “Daddy Do Over” for Dad Bods
/in Eyelid surgery /by adminMen’s Plastic Surgery – “Daddy Do Over” for Dad Bods –
You’ve heard of Mommy Makeovers. How about a Daddy Do-Over? Recent studies report that there has been almost a 30% increase in plastic surgery among men since 2000.
Much like how a group of cosmetic procedures have been embraced by women throughout the country as the so-called “Mommy Makeover,” more men than ever are now turning to their own set of cosmetic treatments to improve their physical appearance in a way that boosts their confidence. Appropriately called a “Daddy Do-Over,” the trend has demonstrated a positive upturn in the number of men embracing the benefits of cosmetic surgery. According to the American Society of Plastic Surgeons, more than 1.3 million procedures were performed on men last year, demonstrating a increase of 29% since the year 2000.
In this article, we will discuss just what a Daddy Do-Over can entail, and how board-certified plastic surgeon Dr. Ali can help “daddies” in the Troy, MI area get the “do-over” they desire.
A Daddy Do-Over Can Improve Confidence
The treatments that make up the Daddy Do-Over include body contouring and facial procedures that can be both surgical and non-surgical. Much like their mommy-targeted counterpart, the hallmark of a Daddy Do-Over is multiple cosmetic procedures being performed during a single visit. The increasing popularity of the Daddy Do-Over trend has seen a huge surge in men turning to medical fat loss treatments as a means of avoiding the divisive physique known as the “dad bod.”
While men obviously do not experience the physical changes that women experience during the period of pregnancy and post pregnancy, their lifestyle does often change in a way that impacts their physical appearance. New parents of every gender are going to get far less sleep than they’re used to, and with young children in the house, it can become difficult to stay on top of consistently healthy patterns of diet and exercise. Brought about by both age and parenting, many men feel that the body they inhabit in their 30s and 40s is barely recognizable. But thanks to the Daddy Do-Over, men are given a means of regaining the level of confidence they had before parenthood.
Daddy Do-Overs to Lose Fat & Inches
Whether or not a man has children, stubborn fat around the midsection becomes increasingly difficult to lose the older he gets. When diet and exercise just aren’t cutting it anymore, many men choose to include a form of medical fat loss in their Daddy Do-Over.
Getting in shape can be a difficult and frustrating task at any age, and only becomes more difficult later in life. But thanks to the reduced stigma surrounding men who undergo plastic surgery, more men than ever are able to achieve their desired aesthetic goals through liposuction, tummy tuck or non-invasive CoolSculpting packaged in our Daddy Do-Over.
Daddy Do-Overs Growing in Popularity!
The popularity of the Daddy Do-Over trend contributed to the more than 200,000 cosmetic surgical procedures performed on men in 2018. The year saw more than 52,000 rhinoplasty procedures performed on men alone, making it the most popular cosmetic procedure of the year among men. The year also saw increases in the number of men undergoing eyelid surgery, liposuction, and male breast reduction. The frequency with which these procedures are performed simultaneously during a Daddy Do-Over supports the claim that the trend played a major roll in the number of men who chose to undergo plastic surgery in 2018
Additionally, the cosmetic procedures that make up a Daddy Do-Over can help improve a man’s confidence in a professional environment. With many places of business placing increased value on the youth of their appearance, many men in their middle ages onward have turned to cosmetic procedures to feel more at home in a work environment composed mainly of people at least a decade their junior.
2018 saw nearly half a million Botox procedures last year, and more than 100,000 reduced the signs of aging with filler injections. With both methods of treatment being popular components of a Daddy Do-Over, the trend has helped boost the confidence of countless men both in and out of the workplace.
Now more than ever, the popularity of the Daddy Do-Over concept is growing, and men are embracing cosmetic surgery more than ever. And in the hands of a board-certified plastic surgeon such as renowned Troy, Mi area cosmetic surgeon Dr. Ali, men of any age have the ability to regain confidence they haven’t felt for many years.
Daddy Do-Over – Troy, MI
While the Mommy Makeover may have gained popularity first, the Daddy-Do-Over is quickly catching up. Whether or not a man has children, the Daddy-Do-Over is an excellent way for men of a certain age to regain the confidence that may have begun to elude them with age. Daddies in the Troy, MI area who are ready for a do-over are encouraged to contact us today for a consultation.

Asian Blepharoplasty – Eyelid Lift Surgery for Asians
/in Blepharoplasty, Eyelid surgery /by adminAsian Blepharoplasty – Eyelid Lifts Surgery for Asians –
Whether to improve one’s vision or simply for cosmetic reasons, one of the most commonly requested procedure by our Asian patients is an upper eyelid surgery known as Asian Blepharoplasty. Asian Blepharoplasty is performed to enhance the fold of the upper eyelid, thus creating a smooth -but still natural-looking- double eyelid
In this article board certified plastic surgeon discusses blepharoplasty as it applies to the unique eye shape of Asian patients.
Asian Cosmetic Upper Eyelid Blepharoplasty
There is no single “correct” or “perfect” eyelid appearance or shape. Beauty truly is in the eye of the beholder, and any type of cosmetic surgery is designed to help the patient feel happier with their own appearance.
Upper eyelid blepharoplasty can be performed for “crease formation”—which is the desire of many Asian patients seeking upper eyelid surgery.
During one’s Asian Blepharoplasty consultation, Troy, MI board-certified plastic surgeon Dr. Ali will take into account any pre-existing fold or crease of the eyelid, as the upper eyelid’s total amount of skin, where the eyebrows are positioned on the face, and most of all, the patient’s own aesthetic goals and desired outcomes of their Asian Blepharoplasty procedure.
The goal is typically NOT at all to “Westernize” the appearance of the Asian eyes. Rather, Asian blepharoplasty is generally performed to obtain a more open and attractive eyelid appearance – while still maintaining the ethnic integrity of the patient’s appearance.
Asian Cosmetic Lower Eyelid Blepharoplasty
Since the bone structure of the typical Asian face has a flatter eye area, Asian individuals, in many cases, may experience very prominent under eye bags as part of the aging process.
Consequently, many cases of Asian Blepharoplasty are also performed on the lower eyelid are done so in order to smooth out such bags.
It is possible to perform Asian Blepharoplasty on the lower eyelid without creating a visible incision. The end result is an under eye area free from bags or unwanted puffiness.
Asian Medical Blepharoplasty for Vision
Asian Blepharoplasty may also be performed to remove redundant skin that obstructs the patient’s line of vision. Many people of all ethnicities experience drooping of the eyelids with age, which can obstruct the vision.
Because of their ethnic eyelid structure, mature Asian patients may be particularly prone to this frustrating issue. However, blepharoplasty performed by an experienced board certified plastic surgeon like Troy, MI area Dr. Ali can lift the sagging eyelids to improve vision – with the added benefit of a more youthful and refreshed appearance.
With documentation from an ophthalmologist diagnosing obstruction to the patient’s field of vision, Asian Blepharoplasty patients may also be entitled to have their procedure covered by insurance. Check with your insurer, as coverage will fluctuate from patient to patient.
The staff at our Troy, MI area medical spa are happy to discuss any concerns regarding insurance with all of our Asian Blepharoplasty patients. In some cases, patients may pay for the procedure prior to the surgery and then may be reimbursed by their insurance carrier at a later date. And for patients undergoing Asian Blepharoplasty for purely cosmetic purposes, financing options are available.

Blepharoplasty FAQs (Part 2)
/in Blepharoplasty, Eyelid surgery /by adminBlepharoplasty FAQs (Part 2) –
Blepharoplasty is eyelid surgery that involves tightening and sculpting the upper and sometimes lower eyelids in order. It can be purely cosmetic, to restore a youthful appearance to the eyes. Or it can be medically motivated to improve vision, when sagging upper eyelids obstruct the ability to see.
In this two part article board certified plastic surgeon Dr. Ali answers patients most frequently asked questions about blepharoplasty eyelid surgery.
Does blepharoplasty get rid of lines around the eyes?
While browlifts are usually implemented to address hooding of the skin to the sides of the eye, upper blepharoplasty is not performed to treat lines and wrinkles. That being said, when performed in conjunction with other effective treatments such as Botox, upper blepharoplasty patients can achieve such results.
Are blepharoplasty results permanent?
Blepharoplasty patients are able to enjoy permanent results, with most patients never requiring another surgery. In some cases, patients returning to our practice for another procedure may also choose to have their blepharoplasty touched up, but this generally occurs as long as 15 to 20 years after the initial procedure.
The worst mistake an unskilled blepharoplasty surgeon will make is removing too much skin, which can result in chronic dryness of the patient’s eyes. Patients who undergo blepharoplasty and then later ask to remove more skin generally require a brow lift, rather than more skin removal.
But in the hands of a board-certified blepharoplasty expert like Dr. Ali, blepharoplasty patients can rest assured that more than enough skin will be left behind for normal, comfortable closure of the eyes.
Successful blepharoplasty makes the patient’s eyes look more open, but does nothing to change the shape or structure of the eyelid. At Oakland County, MI’s AMAE Med Spa, we pride ourselves on having our patients leave all of our blepharoplasty procedures simply looking like younger, fresher versions of themselves.
What anesthesia is used for blepharoplasty?
It is possible to perform upper blepharoplasty with either IV sedation, local anesthesia, or a light general anesthetic. The form of anesthesia chosen depends on whether the patient is undergoing other procedures with their upper blepharoplasty, such as brow lifts or rhinoplasty. Board-certified blepharoplasty expert Dr. Ali helps all of his patients chose the method of anesthesia that is right for them.
Can I wear my glasses or contacts after blepharoplasty?
Blepharoplasty patients are free to pop their glasses back on immediately after their procedure. When exiting the procedure, many blepharoplasty patients chose to mask the swelling around their eyes with sunglasses.
That said, blepharoplasty patients should wait at least a week before wearing contact lenses.
Does insurance cover an upper blepharoplasty?
When obstructions in a patient’s field of vision are documented by an ophthalmologist, they are typically entitled to insurance coverage for their blepharoplasty surgery. That being said, it is important to know the full criteria upon which your insurance company bases their coverage, as insurance companies are famously inconsistent in this regard.
Blepharoplasty patients are encouraged to discuss any concerns regarding insurance with our Oakland County, surgical staff, who will gladly help clarify any such issues. We also offer convenient financing options for patients who are seeking blepharoplasty for strictly cosmetic purposes.

Recovery after Blepharoplasty Surgery
/in Blepharoplasty, Eyelid surgery /by adminRecovery after Blepharoplasty Surgery –
During blepharoplasty, the upper (and occasionally, lower) eyelid is sculpted and tightened through surgery. Blepharoplasty may be performed for entirely cosmetic purposes, such as to create a more youthful appearance for one’s eyes. Blepharoplasty may also be performed for medical reasons, such as to improve vision in patients who have experienced sagging in their upper eyelids severe enough to obstruct their vision.
In this article board certified plastic surgeon Dr. Ali answers explains what patients can expect during the recovery period after blepharoplasty eyelid surgery.
Going Home after Blepharoplasty
Blepharoplasty patients are not required to stay overnight at the hospital unless the surgery is being performed in conjunction with an additional procedure, such as a facelift. Blepharoplasty patients must, however, be driven home from their surgery by someone else and should have someone stay with them for the night.
Pain after Blepharoplasty
Any discomfort is typically well managed with over the counter medications such as Tylenol.
Patients who experience intense pain may be prescribed pain medication by Dr. Ali, but such instances are uncommon, with blepharoplasty not being a particularly painful procedure. Any discomfort is typically well managed with over-the-counter medications such as Tylenol.
For the first several days of blepharoplasty recovery, patients should apply cold compresses to the treatment area, which will both reduce swelling and bruising and relieve the patient from pain and discomfort. While cold compresses work wonders to aid in the healing process, blepharoplasty patients should not apply ice directly to the eyelids.
Glasses and Contacts after Blepharoplasty
Not only is it possible for eyeglasses to be worm immediately after the procedure, when patients return the day after their surgery for their follow-up, the frequently arrive wearing sunglasses to mask the swelling caused by the procedure.
While blepharoplasty patients are in the clear to wear glasses almost immediately after treatment, patients should refrain from wearing contact lenses for at least a week after their surgery.
Returning to Work after Blepharoplasty
For blepharoplasty patients who work from home whose daily routine involves non-strenuous tasks such as working on the computer or over the phone, it is possible to return to work the day after their surgery.
But blepharoplasty patients will have stitches in the treatment area for a week after their procedure, and will often experience swelling and bruising around their eyes. For patients who do not mind being seen in public in such a state, it is possible to return to work once three days have passed. For everyone else, it is advised to plan on taking off about a week from work.
Driving after Blepharoplasty
Blepharoplasty patients may resume driving as soon as they are completely off any kind of narcotic pain medication and no longer feels at all restricted by their discomfort in a way that would impede their ability to do so.
Exercise after Blepharoplasty
Most patients report feeling ready to resume their exercise routine about a week after their blepharoplasty surgery. However, in some extremely rare cases, exercise has occasionally resulted in bleeding in some patients up to two weeks after their surgery. For this reason, board-certified plastic surgeon Dr. Ali recommends that all of his patients wait at least three weeks following their blepharoplasty procedure before diving back into their workout routine.
Showers after Blepharoplasty
Unless Dr. Ali has instructed the patient to refrain from doing so in their aftercare instructions, blepharoplasty patients are able to take a shower the night of their upper blepharoplasty surgery.
Postoperative Care after Blepharoplasty
Generally, the incision made during blepharoplasty does not require special care. For the post part, it is alright to get the eyelid wet immediately following surgery, such as by cleaning it with a soft washcloth. Once the sutures have been removed, blepharoplasty patients do not need to apply any kind of special scar ointment to the treatment area, as the healing properties of the eyelid rank among the best in the human body.
4-6 days after blepharoplasty, the sutures will be removed.
While it is exceedingly rare, it is possible for patients to experience bleeding that may apply pressure to the eye. If left untreated, this incredibly uncommon occurrence could theoretically result in blindness. Patients should immediately call their doctor if their eye become swollen after their blepharoplasty surgery, or if they experience severe pain or loss of vision. If they do not immediately receive a response, the individual should go to the emergency room. But, once again, this is exceeding rare and due to his skill and experience it has not happened to any of Dr. Ali’s patients.

Recovery after Gynecomastia Surgery
/in Male Breast Reduction /by adminRecovery after Gynecomastia Surgery –
Gynecomastia, or male breast enlargement, affects as many as 50% of men at some point in their lives. While some cases may be temporary, many men elect to undergo gynecomastia surgery, also known as male breast reduction.
In this article Warren, MI area board certified plastic surgeon Doctor Ali explains what to expect during the recovery period after undergoing gynecomastia (male breast reduction) surgery.
Going Home after Gynecomastia Surgery
Gynecomastia surgery is generally performed as outpatient surgery, meaning that you can go home after you are discharged from recovery. Because the surgery involves anesthesia, you will need to have someone drive you home, and it is also very helpful to have someone stay with you at least for the first night or two, as you will be a bit groggy for a while. Most patients are able to return to work as early as 3 to 7 days post-surgery, depending on what the job entails. Exercise can be resumed gradually, but strenuous exercise or job duties should not be resumed until you get the all-clear from your doctor.
Pain after Gynecomastia Surgery
While the amount of pain and discomfort varies greatly, most men report that they feel the type of pain associated with a vigorous workout, while others liken the pain to being punched in the chest. Upon discharge from your gynecomastia surgery procedure, you will be given narcotic pain pills. However, many men say they didn’t need them, but rather simply used over the counter pain relief medications such as Tylenol.
Drains after Gynecomastia Surgery
Unless your gynecomastia surgery involves removing very large amounts of breast tissue, it is unlikely that you would be sent home a post-surgical drain. Dr. Ali will determine prior to your surgery whether or not a drain will be necessary, and if it is, he will give you detailed instructions on how to deal with the drain as you recover.
Swelling after Gynecomastia Surgery
It is common to experience some degree of swelling after surgery, which may increase for about a week. After that, the swelling will reduce quickly, and then gradually tapers off until after a month, it is hardly noticeable. However, the swelling will continue to reduce for the next three months up to even a year, although the changes during that period of time are very subtle.
Compression Vest after Gynecomastia Surgery
Before you leave the operating room, you will be fitted with a compression vest, which is a vest fitted with foam underneath, to provide support and to reduce swelling. After a few days, you will be instructed to remove the vest and throw away the foam. You should still wear the vest when you are recovering around the house. When you go out, you can wear a snug Under Armor-type of shirt. While there is no hard and fast rule as to how long you should wear the shirt or vest, the longer you wear it, the faster your swelling will subside. A good rule of thumb is to wear the compression vest for about a month.
Bathing after Gynecomastia Surgery
Dr. Ali suggests that you wear the initial post surgical garment for two days. After that, you may remove it to take a shower. However, you will need to put the compression vest back on afterwards.
Returning to Work after Gynecomastia Surgery
Depending on what your job entails, you can expect to return to work about three days after your gynecomastia surgery. Of course, if your job involves heavy lifting or strenuous activity, you will need to wait a bit longer. And conversely, if your job entails light duties, you may be ready to returning even sooner.
Driving after Gynecomastia Surgery
Most patients feel up to driving after about three to five days. Before getting behind the wheel, you should be confident that you feel completely unrestricted by your discomfort, and feel secure in making any abrupt movements that may be necessary as you drive your car. Of course, as long as you are taking narcotic pain medication, you must avoid driving entirely. Ultimately, you may resume driving when you are confident that you can do so safely.
Exercise after Gynecomastia Surgery
Depending on the extent of your surgery, you may resume exercise after about three weeks to a month. Dr. Ali can tell you when it is safe to begin your exercise regimen, and can suggest some intermediary steps that involve movement, but less stress to the surgical site.
Sun Exposure after Gynecomastia Surgery
During your recovery, it may sound like a good idea to lie out in the sun and relax. However, you should only do so if your incision is protected so that it is completely blocked from the sun. In fact, it is best to keep your incision blocked from the sun’s rays for at least a year.

Gynecomastia Surgery FAQs (Part 1)
/in Male Breast Reduction /by adminGynecomastia Surgery FAQs (Part 1) –
Gynecomastia surgery – also called male breast reduction surgery – is a type of plastic surgery to remove excessive or overgrown glandular breast tissue from men and sometimes teenage boys.
In this article Oakland County board certified plastic surgeon Dr. Ali answers patients most frequently asked questions about Gynecomastia surgery.
What is gynecomastia?
Gynecomastia is a common condition in men and boys where the breasts swell and become abnormally large. It is generally caused by a hormonal imbalance (either naturally occurring or from steroid use) that causes the growth of glandular tissue. Gynecomastia affects an estimated 80% of adolescents, but in normal cases, goes away on its own. However, in some cases it doesn’t, requiring surgical intervention. In mild cases, there is just a bit of protrusion; but in more severe cases, the male chest takes on the appearance of female breasts.
This can be very embarrassing to men, and some are hesitant to go to the beach or to take off their shirts in public. Many men find that their social lives are negatively impacted, and eventually seek medical advice. The good news is that gynecomastia can in most cases be corrected with surgery.
What is pseudogynecomastia?
Overweight men may develop an overabundance of chest fat, and even after losing weight, the fat in the chest area may remain. This condition is called pseudogynecomastia as opposed to true gynecomastia. True gynecomastia involves glandular tissue, as opposed to fat.
What causes gynecomastia?
There are many things that can cause gynecomastia, including hormonal level changes, some medications, heavy alcohol use, recreational drugs, some herbal products, various health imbalances, and excessive weight gain.
Pseudogynecomastia, on the other hand, is simply extra fat that has found its way to the chest area. Pseudogynecomastia can be caused by aging, weight gain, hereditary factors, or other non-medical issues.
Are there other treatments besides surgery for gynecomastia?
If you are overweight, losing the extra pounds may help, especially if you have pseudogynecomastia. In this case, liposuction or CoolSculpting may be able to remove remaining excess fat.
If you have true gynecomastia, however, losing weight may not necessarily help, because the chest enlargement is caused by excess glandular tissue. This firm and rubbery tissue cannot be removed by liposuction, and generally, surgery is the only option.
How is gynecomastia surgery performed?
Gynecomastia surgery involves surgical excision, which means the glandular tissue is cut out. The procedure is normally done using a general anesthesia, IV sedation, or local anesthesia, depending upon your physical anatomy and preference. A small incision is made in the lower part of the areola and the excess tissue is taken out through the incision. Dissolving stitches are used to close the incision. Normally, no drains are necessary, although in some more involved scenarios, you may be sent home with drains in place that will help your body eliminate excess fluid and blood.
Dr. Ali will discuss anesthetic options with you. Many patients prefer a general anesthetic, under which they are totally asleep. However, in many situations, IV sedations can work equally as well.
Are the results of gynecomastia surgery permanent?
While the surgical procedure is considered to be “permanent,” in some rare instances, gynecomastia can recur. For instance, if the gynecomastia was caused by steroids and the patient continues to use them after surgery, gynecomastia could easily reoccur. In cases of pseudogynecomastia, if the patient receives liposuction and then regains the weight, it may be necessary to repeat the procedure, or to opt for the surgical procedure that will yield more thorough results.

Gynecomastia Surgery FAQs (Part 2)
/in Male Breast Reduction /by adminGynecomastia Surgery FAQs (Part 2) –
Gynecomastia surgery – also called male breast reduction surgery – is a type of plastic surgery to remove excessive or overgrown glandular breast tissue from men and sometimes teenage boys.
In this two part article Detroit area board certified plastic surgeon Dr. Ali answers patients’ most frequently asked questions about Gynecomastia surgery.

What age can I get gynecomastia surgery?
Gynecomastia is actually very common among teenage boys, and generally resolves itself in time. Therefore, it is best to postpone surgical discussions until around the age of 18 unless the case is extreme or is having a devastating effect on the teen’s health, wellbeing or self-esteem.
There is no upper age limit for gynecomastia surgery, as long as the patient is in good health and does not have any underlying bleeding disorders or other risk factors. Dr. Ali has helped many senior men regain a firm, masculine and youthful chest with gynecomastia surgery.
What is the goal of gynecomastia surgery?
The best outcome for a successful gynecomastia surgery is a chest that has a natural contour. It is normal for there to be some glandular tissue beneath the areola. Therefore, the surgeon will remove some of the fat and glandular tissue, but will retain enough so that the chest retains a nice shape.
If the surgeon were to remove too much fat, while the chest may look flat when the man’s arms are at his side, when he raises his arms there could actually be a depression underneath the areola.
One’s overall shape also needs to be taken into consideration. For instance, if a man is overweight with a prominent abdomen, removing all or most of the fat from the chest will create a result that is completely out of proportion.
Will I have scars from gynecomastia surgery?
If glandular tissue is removed, scarring will occur on the lower perimeter of the areola. Dr. Ali is a skilled plastic surgeon, and is extremely careful in closing the incisions so that any scarring is minimal. In time, these scars will fade, and will eventually be almost unnoticeable.
In the case of liposuction, scarring is limited to two incisions that are quite small and normally fade away almost completely.
Hair on their chest can help to conceal any scars that remain. However, men universally prefer to have a couple tiny scars, in exchange for getting rid of feminine or oversized breasts!
Can I have other procedures done at the same time as gynecomastia surgery?
Many men choose to address excess fat in the stomach area and/or “love handles” through liposuction at the same time they undergo gynecomastia surgery. Dr. Ali can discuss these options with you at the time of your consultation, and decide what is best for you in terms of your goals and your overall health.
Does insurance cover gynecomastia?
Each insurance company has different policies and procedures, but in general, if gynecomastia is considered purely cosmetic, insurance companies most likely will not pay. There are some instances where gynecomastia surgery may be covered if the condition is extremely disfiguring or contributes to or causes other health issues.
Is there downtime after gynecomastia surgery?
Gynecomastia surgery is normally performed as an outpatient procedure, which means that you will go home after your procedure. You will need someone to drive you home, as you will have been under anesthesia; and Dr. Ali recommends that you have someone stay with you for the first day or two. Most patients report that they feel up to returning to work after three days to a week, assuming their job duties are not too strenuous. Dr. Ali will let you know when it is safe to resume your exercise regimen, but likely you will be ready after about 3-4 weeks.

Who Is a Good Candidate For Gynecomastia Surgery?
/in Male Breast Reduction /by adminWho Is a Good Candidate For Gynecomastia Surgery?
True gynecomastia occurs when an imbalance of hormones results in the enlargement of breast tissue in the male chest. The hormonal imbalance may either occur naturally or be stimulated through the use of steroids such as those taken by bodybuilders. Weight gain can also affect the amount of fat cells that locate in the chest area, and in some cases, the enlarged chest may persist even after the weight is lost.
If the enlarged chest is caused by a buildup of glandular tissue, or a combination of glandular tissue and fat, gynecomastia surgery may be the best option for treatment.
In this article, Dr. Ali, Macomb County board certified plastic surgeon, discusses gynecomastia surgery, who is a good candidate for it, and alternatives to the surgery.
Gynecomastia Surgery for Overweight Patients
While gynecomastia is not a weight loss method, many patients undergoing gynecomastia surgery are overweight. Gynecomastia will typically remove several inches from the chest in addition to tightening and contouring it.
Some men elect to undergo gynecomastia surgery before losing weight because it gives them a good jump start to their weight loss efforts. While the surgery will address their feminine-looking breasts, care must be taken to make sure the chest remains in proportion to the abdomen. If the chest is made flatter but the abdomen still protrudes because of excess fat, the appearance overall will look unnatural and unbalanced.
Gynecomastia Surgery for Teenagers
Gynecomastia is actually quite common in adolescents, and generally subsides by the late teens. In most situations, doctors prefer to wait until after the age of 18, to see if it resolves naturally.
However, sometimes the enlarged chest is so pronounced that it requires medical intervention. Additionally, severe gynecomastia can be the source of profound embarrassment for a teenaged boy, and it may affect his social life and self-esteem. Simple, normal activities such as going to a pool party, playing sports or even changing in the locker room can cause undue amounts of anxiety for a teen suffering from gynecomastia.
Alternatives to Gynecomastia Surgery
If excess fat is the cause, gynecomastia can be treated effectively in many cases with liposuction. While liposuction doesn’t remove glandular tissue, it does effectively address excess fat. Plus, the recovery is quicker.
CoolSculpting is another option, and is available in our Macomb County medical spa. A special machine freezes the areas of the breast that contain excess fat, and over the next couple of months, the fat is eliminated naturally by the body. However, CoolSculpting does not work for true or hormonal gynecomastia.
Deciding to Get Gynecomastia Surgery
Only you can decide if your excess chest tissue or fat bothers you to the extent that you are willing to undergo a surgical procedure to get rid of it. If you are embarrassed to take your shirt off by the pool or at the beach, or if you are uncomfortable in tight-fitting or thin shirts because your chest appears to be more feminine than masculine, you may want to schedule a consultation to explore your options.

Nipple Reconstruction Surgery FAQs
/in Breast Reconstruction, Nipple Correction Surgery /by adminNipple Reconstruction Surgery Frequently Asked Questions–
Dr. Ali is a prominent board certified plastic surgeon in the greater Birmingham, MI area, with decades of experience and thousands of satisfied patients. With over 20 years in practice and extensive training, Dr. Ali is a Fellow in the American College of Surgeons, and has been rated Top Doc by Hour Detroit Magazine multiple years in a row. In this article, Dr. Ali discusses nipple reconstruction surgery, and answers some frequently asked questions about the nipple reconstruction procedure.
How is Nipple Reconstruction Surgery Performed?
Most often, the nipple is constructed from a flap of skin on the top of the breast, such as what is called the C-V and skate flaps. These procedures can be performed as an outpatient. The scar-based modified C-V flap can be performed for nipple reconstruction and scar revision at the same time.
Reconstruction of the areola is performed using a full-thickness circular skin graft from a different location on the patient’s body, for instance, the bra roll, a C-section scar, or even sometimes the inner thigh. Using a skin graft allows the areola to maintain a distinct topography from the rest of the breast, and it also enables the physician to hide some of the scars left by the mastectomy.
Does Nipple Reconstruction Surgery Require Hospitalization?
Nipple reconstruction surgery is almost always performed on an outpatient basis, meaning that a hospital stay is not necessary.
What Type of Anesthesia is Used for Nipple Reconstruction Surgery?
Generally, nipple reconstruction surgery is performed using a local anesthetic. Dr. Ali will inject a medication that numbs the area where the reconstructed nipple will be located. Under a local anesthetic, you will be awake during the procedure, although you will likely be given a medication to allow you to relax.
In the event Dr. Ali feels that a general anesthesia would be preferable, you will be given an IV (intravenous infusion), which would be inserted into a vein in your hand or arm and taped securely in place. Along with the anesthesia, a medication to relax you is inserted through the IV line. With this type of anesthesia, you will be asleep during the procedure.
Can I have Nipple Reconstruction Surgery During Chemotherapy?
Usually, nipple reconstruction surgery is the final stage of breast reconstruction. Chemotherapy is usually completed before breast reconstruction is commenced. Most doctors want the patient to be fully recovered from their mastectomy and effects of chemotherapy before beginning the reconstruction process.
Are There Non-Surgical Alternatives to Nipple Reconstruction?
Nipple tattoos are becoming an increasingly popular option for women who have undergone breast reconstruction surgery after cancer. Techniques have improved to the point where it is difficult to tell the difference. Dr. Ali can refer you to a specialized nipple tattoo expert who can create extremely natural-looking 3D tattoos using medical tattooing.
How Long is Recovery from Nipple Reconstruction Surgery?
Recovery time depends upon the technique selected. If you decide upon a tattoo, you can return to work the very next day.
The surgical flap technique is a little bit more involved, and the length of recovery depends upon the location of where the skin graft is taken. Generally, the area around the newly-constructed nipple and areola are for the most part insensate, so most patients report minimal discomfort.
When the flap technique is used, patients normally take a few days for the initial healing and then can return to work. However, you should wait from two to four weeks before intense exercise is resumed, in order for the surgical scars to heal properly. In the meantime, gentle exercise such as walking is encouraged, and you can begin to walk as soon as the evening of the procedure if you feel up to it.
Is Nipple Reconstruction Surgery Permanent?
While nipple reconstruction surgery is considered to be permanent, the newly created nipples may flatten a bit after a length of time, and tattoos can fade somewhat. Additional procedures down the road to maintain the projection or color are not unusual, and are happily performed by our Birmingham surgical team – to keep your breast and nipples looking beautiful and natural.
Will My Insurance Cover Nipple Reconstruction Surgery?
If you are having reconstruction done post-mastectomy, both surgical and non-surgical options for nipple and/or areola reconstruction are generally covered by insurance companies.
While this article answers many questions you may have, if you are considering surgery to reconstruct your nipples, no doubt you have more. It is important that you select a surgeon in whom you have confidence, who will take the time needed to explain all of your options and put you at ease, and with whom you feel completely comfortable. If you live in or near the greater Birmingham, MI area and are considering nipple reconstruction surgery, schedule a consultation with Dr. Ali to discuss your unique situation and the best options available to you.
Typically nipple tattoo experts do not take insurance, but do provide information to the patient so that they can submit to their insurance for reimbursement.
