Silicone Breast Implant Regulation: 1990s
In 1992, the FDA still had concerns that the breast implant manufacturers were not adequately addressing public concerns about certain complications. At that time a higher incidence of implant rupture and silicone leakage was being seen.
So, the FDA removed all silicone breast implants from sale in the US, and required all manufacturers to submit premarket approval applications that contained detailed data on safety and effectiveness.
However, in the meantime, the FDA did allow manufacturers to provide silicone breast implants for reconstruction after mastectomy, correction of congenital deformities, or replacement of existing implants. So, the manufacturers enrolled women who received silicone breast implants for medical (not cosmetic) purposes in performance and safety studies.
Then in 1999, the Institute of Medicine (IOM) released a detailed report compiling the results of all of these published and ongoing studies on breast implants, called Safety of Silicone Breast Implants.
This report concluded that there was no evidence that silicone breast implants caused systemic health effects such as cancer or autoimmune disease. And while it did identify rare local complications (which included rupture, pain, capsular contracture, disfigurement, infection, and repeat surgeries) silicone breast implants were now recognized as generally safe.
Silicone Breast Implant Regulation: 2000s
After years of studies and technological improvements, the FDA approved silicone gel-filled breast implants in November 2006.
As with any surgery, there is always a small number of possible local complications and adverse outcomes. However, the FDA concluded that the benefits and risks of breast implants were sufficiently well understood for women to make informed decisions about their use.
Initially, in 2006, the FDA only approved Mentor’s MemoryGel Silicone Gel-Filled Breast Implants and Allergan’s∗ Natrelle Silicone Gel-Filled Breast Implants – followed six years later by Sientra’s Silicone Gel Breast Implants.
Silicone Breast Implant Regulation: 2010s
Advanced technologies in breast implant look, feel and safety continued to be developed. And in 2013 Mentor’s MemoryShape and Allergan Natrelle 410 Highly Cohesive Breast Implants were approved.
Today these five breast implants remain the only 5 breast implants that are FDA approved for use in the US.

Silicone Breast Implant Regulation: 1960s
Gel-filled silicone breast implants were introduced to the U.S. in 1962, and in common use by 1964.
This new technology was imperfect at first. In large part this was due to lack of study or regulation on this new technology.
But thanks to advancements in technology, rigorous scientific testing, and intensive regulation by the FDA, silicone breast implants are now very safe.
Silicone Breast Implant Regulation: 1970s
Before 1976 breast implants were not highly regulated. In 1976 the U.S. Congress passed the Medical Device Amendments to the Federal Food, Drug, and Cosmetic Act. Under this law, breast implants were considered “Class II” (moderate risk) medical devices – and as such they had to comply with general controls and performance standards.
At that time the FDA reviewed new breast implants through a process called the 510(k) Premarket Notification process, like any other Class II products.
Silicone Breast Implant Regulation: 1980s
In the early 1980s, the FDA began to identify certain local complications and adverse outcomes of these older silicone breast implants. Some reports even described cancer and connective tissue disease in a small number of women with breast implants.
As concerns arose about the safety of silicone breast implants rose, the FDA reclassified breast implant into “Class III” (higher-risk), which requires premarket approval. Breast implant manufacturers now had to provide data demonstrating the breast implants were safe and effective BEFORE they could be sold.

Protecting Your Breast Implants
After you have healed from breast implant surgery, normal activity, exercise and sexual intimacy is not only recommended, but encouraged, as part of a healthy lifestyle!
You will want to guard your implants against any kind of intense or repetitious pressure on your breasts. While you should be able to engage in normal physical activities such as sports or exercise, you will need to avoid any kind of situation that could result in any kind of trauma to your breast. Detroit Board Certified Plastic Surgeon Dr. Ali will gladly advise you of any activites he believes you should avoid following your breast implant surgery.
Following breast implant surgery, make sure to let all healthcare practitioners (doctors, mammogram technicians, massage therapists, chiropractors, acupuncturists, physical therapists, nursis, etc.) treating you know that you have undergone silicone gel breast implant surgery.
Failure to do so could result in accidental damage or rupturing of your breast implants. All Detroit area patients who undergo breast implant surgery should carry their Device Identification Card with them at all times to show to healthcare practitioners prior to proceeding with treatment.

Monitoring Your Breast Implants for Rupture
There are numerous factors that can result in your breast implants developing a hole or tear in their shell. For the most part, these holes are referred to by breast implant surgeons as ruptures because they allow for silicone gel from inside the implant to secrete out of the implant.
If one or both of your implants rupture, you may find yourself experiencing a variety of symptoms. Talk to Detroit Board Certified Breast Implant Surgeon if you experience any of the following symptoms, as they may be a result of your implant(s) having ruptured: changes in breast size or shape, tingling, pain, numbness, swelling, hardness of the breast, or if you notice lumps or hard knots surrounding the implant or in the armpit.
It is possible, however, that you may experience an implant rupture without experiencing any symptoms at all. In such cases (what are known as “silent ruptures,”) it can be more difficult to determine if you have experienced a rupture. Talk to Detroit Board Certified Breast Implant Surgeon Dr. Ali if you notice anything that feels out of place about your breast implants.
Detroit Board Certified Breast Implant Surgeon Dr. Ali will most likely prescribe an MRI examination to determine if a silent rupture has occurred. As MRI’s are the best identifier of silent ruptures, it is strongly recommended that you arrange for an MRI with your breast implant surgeon in the third year following your surgery, then once more every two years for as long as you have your breast implants.
It is the opinion of Detroit Board Certified Breast Implant Surgeon that patients with both saline filled and silicone gel-filled breast implants should arrange for routine mammography for breast cancer screening, based on age and risk factors.
What to do if you Suspect Breast Implant Rupture
If you believe that the silicone gel has moved out of your breast implant or that your implant has ruptured, call Detroit Board Certified Breast Implant Surgeon Dr. Ali right away to schedule an exam. The best way to diagnose a rupture is through an MRI.
If Detroit Board Certified Breast Implant Surgeon Dr. Ali confirms that your implant has leaked or ruptured, he will consult you on all of your options, which may involve the removal and replacement of your breast implant, depending on your medical needs.

Once you’ve booked your breast augmentation surgery procedure, you’ll probably feel both excited and a little bit nervous! After all, the body you’ve always dreamed of is on its way!
One area some patients are sometimers apprehensive about is the idea of “going under” with general anesthesia. In fact, recent studies report that nearly one in three people worry about the anesthesia more than the surgical procedure itself. Approximately 55% of those undergoing surgery are concerned about general anesthesia.

General Anesthesia is Very Safe During Breast Augmentation Surgery
Thanks to modern pharmaceuticals, and the the latest technologies in anesthetic administration, the risk of general anesthetic complications has decreased more than 90% in the last 50 years.
Complications with anesthesia dropped more than half from 2010-2013. And mortality rates from anesthesia remain inceredibly low, at only 0.03% of all surgeries. And it is important to remember that these numbers all take into account people who were in poor health to begin with, as well as those who’d had surgeries done in emergency situations.
“If you’re approved for cosmetic surgery,” notes Detroit area plastic surgeon Dr. Ali, “chances are you’re in good health already. This greatly improves your chances of a favorable outcome with general anesthetic.”
How to Prepare for General Anesthesia in Breast Augmentation Surgery
Properly preparing for your breast augmentation surgery –and general anesthesia—helps to avoid unnecessary complications.
Be sure to tell your doctor all medications you’re taking, including herbal supplements or remedies. Many of these can interact with anesthesia medications, so it’s crucial that you’re thorough and completely honest with your doctor. You also need to fully disclose any medical conditions or procedures you may now have. or have had in the past.
If you take any ongoing medications, including blood thinners and those for diabetes or blood pressure, Dr. Ali will discuss any changes you may need to make the day of the surgery, as well as in the weeks before.
You will be given specific pre-surgery instructions, which you should follow carefully. Generaly you will be instructed to fast for six hours or more before the surgery to make sure the anesthesia has the desired effect.
Finally, be sure to arrange a ride home for yourself. You will need someone to drive you back after your surgery, so this should be figured out beforehand.

Board certified Breast Augmentation Surgeon Doctor Ali will decide which type of anesthesia is right for you, based on your current and past health, relevant test results, and the type of Breast Augmentation surgery being performed. He will discuss all of this with you during your consultation. The Day of Breast Augmentation Surgery
You will meet your anesthesiologist on the day of your Breast Augmentation surgery. He or she will constantly monitor your vital signs (heart rate, breathing, blood pressure) before, during, and after the surgery – and will also continuously adjust the levels of anesthesia you’re given. If there are any issues related to anesthetic during the Breast Augmentation surgery, they will correct them by giving additional fluids or medications.
Your anesthesia medications may be given intravenously (IV) or inhaled through a mask or both.
You will be free of pain, consciousness, movement, and memory during the time you’re under general anesthesia. Many people describe their experience as a “time warp,” where they aren’t sure how much time has passed when they wake up after surgery.
How quickly you wake up afterward depends on multiple factors: the type of anesthetics and other medications used, how you responded to them, and what amounts were given.
After your Breast Augmentation surgery, you will go to a recovery room where you’ll continue to be monitored. Your vital signs and bandages will be checked, and you’ll be asked how you’re feeling. If you experience any pain, be sure to tell your medical team so they can help you.

There may be some side effects of anesthesia. These are temporary, but can last for several hours after surgery.

What Is an Inverted Breast Nipple?
An inverted nipple is a nipple that protrudes inward, instead of outward. It can look flat, mishappen, or even appear to be a sunken hole where the nipple would normally be located.
Nipple inversion is more common than most people realize, affecting at least 2% of all men and women. Some people are born with inverted nipples, while others have nipples that become inverted later in life due to breastfeeding, a tumor pulling the nipple inward, trauma or other conditions.

You will be encouraged to begin walking around on the day of surgery, and can return to work as soon as you feel well enough. Many patients return to work as early as a day or two after surgery. Any stitches will generally be removed about 1 to 2 weeks following the procedure.
Patients are advised to avoid strenuous activity as well as sexual activity for a week or two. You should also avoid exposing the resulting small scars to the sun for at least six months. Sunlight can permanently affect the skin’s pigmentation, causing the scar to turn dark. If sun exposure is unavoidable, use a strong sun block.

Breasts are usually a significant source of self-esteem for most women. Women who feel good about the size and shape of their breasts, also tend to feel better about themselves.
Conversely, when a woman is disatisfied with her breasts – whether it’s the size, shape, or position – feelings of frustration and low self-esteem can result. And the pressure put on women today by celebrity standards and social media posts can make a woman even more self conscious about areas of her body that she doesn’t like.
Breast Augmentation
Breast augmentation is one of the most commonly performed plastic surgery procedures. “Augmentation” is simply defines as the process of making or becoming greater in size or amount. So a breast augmentation involves increasing the size of a woman’s breasts. Typically, breast augmentation involves the use of implants to enhance the shape and size of the breasts.
Breast augmentation can help women who feel that their breasts are too small in size, and can also help women who wish to restore their breast’s fullness following pregnancy, weight loss, and aging.

Breast reconstruction surgeries may also follow mastectomies performed for other medical purposes such as the prevention of disease, and can be performed to correct a genetic mutation. In rare cases, burns or other trauma may have damaged or destroyed breast tissue. These situations can also be treated with breast reconstruction surgery.
Breast implants may or may not be needed or desired by a breast reconstruction patient. During a breast reconstruction procedure, the nipple and the darker area around the nipple (called the areola) can also be restored or added to the new breast.
For most patients, breast reconstruction is a physically and emotionally rewarding procedure that improves the patients’s self-esteem, self-image, and quality of life.
