How Breast Surgeons Correct Inverted Nipples

Breast Surgeons Can Correct Inverted Nipples

Many women desire cosmetic changes to their breasts, and one of the most common procedures performed by breast surgeons is correcting an inverted nipple.

Detroit area double board certified breast surgeon Doctor Ali has enhanced, restored and repaired hundreds of women’s breasts, giving them the appearance and the self confidence they desire. In this article he discusses breast inversion correction surgery.

What Is an Inverted Breast Nipple?

An inverted nipple is a nipple that protrudes inward, instead of outward. It can look flat, mishappen, or even appear to be a sunken hole where the nipple would normally be located.

Nipple inversion is more common than most people realize, affecting at least 2% of all men and women. Some people are born with inverted nipples, while others have nipples that become inverted later in life due to breastfeeding, a tumor pulling the nipple inward, trauma or other conditions.

An inverted nipple can cause embarrasment during intimacy, in the locker room or fitting room, or any time a woman’s breast may be seen. In addition to cosmetic concerns, inverted nipples can also sometimes cause functional problems, such as making breastfeeding difficult.

How Breast Surgeons Correct Inverted Nipples

Breast surgeons can perform a simple and routine surgery on the nipples that restores a natural appearance. There is usually no to very little scarring, and the milk ducts are not affected for future breastfeeding.

During inverted nipple breast surgery the surgeon reshapes the nipple and areola so that the nipple projects out from the breast. This changes the appearance of the breast and still preserves the sensitivity of the nipple. Certain procedures also make sure the milk ducts are still working so that women can still breastfeed afterwards.

What to Expect After Nipple Correction Surgery

You will feel some discomfort for a few days after surgery, but this is easily managed with pain medication. After the Nipple Correction Surgery, some bruising and swelling is normal, but this typically only lasts for a few days.

You will be encouraged to begin walking around on the day of surgery, and can return to work as soon as you feel well enough. Many patients return to work as early as a day or two after surgery. Any stitches will generally be removed about 1 to 2 weeks following the procedure.

Patients are advised to avoid strenuous activity as well as sexual activity for a week or two. You should also avoid exposing the resulting small scars to the sun for at least six months. Sunlight can permanently affect the skin’s pigmentation, causing the scar to turn dark. If sun exposure is unavoidable, use a strong sun block.

What Is The Difference Between Breast Augmentation & Breast Reconstruction?

Breast Augmentation vs. Breast Reconstruction

One of the many questions we are asked at Amae Plastic Surgery Center is “What is the difference between breast augmentation and breast reconstruction?” In this article Detroit area board certified plastic surgeon Doctor Ali, explains these two different sugeries.

Breasts are usually a significant source of self-esteem for most women. Women who feel good about the size and shape of their breasts, also tend to feel better about themselves.

Conversely, when a woman is disatisfied with her breasts – whether it’s the size, shape, or position –  feelings of frustration and low self-esteem can result. And the pressure put on women today by celebrity standards and social media posts can make a woman even more self conscious about areas of her body that she doesn’t like.

Breast Augmentation

Breast augmentation is one of the most commonly performed plastic surgery procedures.  “Augmentation” is simply defines as the process of making or becoming greater in size or amount. So a breast augmentation involves increasing the size of a woman’s breasts. Typically, breast augmentation involves the use of implants to enhance the shape and size of the breasts.

Breast augmentation can help women who feel that their breasts are too small in size, and can also help women who wish to restore their breast’s fullness following pregnancy, weight loss, and aging.

Breast implants can be saline or silicone and come in a variety of shapes and sizes. We have several articles in this Blog on breast implant size and shape:

Download our free white papers on breast reconstruction and breast augmentation.

Detroit area board-certified plastic surgeon Doctor Ali will help you determine which type, size and shape of breast implant is best to obtain your desired appearance results.

During breast augmentation surgery, the incision will typically be put in an inconspicuous area (such as the armpit or lower half of the areola) so there is no need to worry about significant scarring.

During breast augmentation surgery, in addition to breast implants, a patient may also desire a “breast lift” to correct sagging skin and raise the breasts upweards to a more youthful looking position.

Breast Reconstruction

Breast reconstruction is a cosmetic surgery procedure that is aimed at restoring the breast(s) as closely as possible to its original appearance and size. This procedure traditionally follows a mastectomy (the surgical removal of one or both breasts) as a way to treat breast cancer.

Breast reconstruction surgeries may also follow mastectomies performed for other medical purposes such as the prevention of disease, and can be performed to correct a genetic mutation. In rare cases, burns or other trauma may have damaged or destroyed breast tissue. These situations can also be treated with breast reconstruction surgery.

Breast implants may or may not be needed or desired by a breast reconstruction patient. During a breast reconstruction procedure, the nipple and the darker area around the nipple (called the areola) can also be restored or added to the new breast.

For most patients, breast reconstruction is a physically and emotionally rewarding procedure that improves the patients’s self-esteem, self-image, and quality of life.

Pregnancy, Breast Feeding and Breast Implants

Pregnancy, Breast Feeding and Breast Implants

Detroit area board certified plastic surgeon Dr. Ali has performed thousands of breast implant surgeries on women before and after pregnancy. Many women wonder the effect of breast implants, if any, on pregnancy and breastfeeding. about In this article Doctor Ali answers your questions about breast implants and pregnancy or breastfeeding.

No Breast Implants for Women who are Pregnant or Nursing

While it seems like “common sense” it is important to note that breast implant surgery should not be performed on a woman who is pregnant or nursing. However, women may get breast augmentation surgery before or after becoming pregnant and/or nursing.

Always ensure you are not pregnant before undergoing breast implants, taking a pregnancy test if there is any doubt or possibility that you may be carrying a child.

Breast Implants Before Becoming Pregnant

Some patients have asked us if having silicone gel breast implants could harm babies whose mothers became pregnant after getting breast augmentation.

We understand that nothing is more important to you than the health of your children! There is no evidence to shows that silicone breast implants have any harmful effects on children born to women with implants.

Two clinical studies in humans have found that there is NO increase in the risk of birth defects in children conceived and born after a women has had breast implant surgery.

Breast Implants Effect on Breast Feeding

In some women, there is a very small possibility that breast implant surgery might interfere with the ability to breastfeed. A very small percentage of women who have had breast augmentation surgery produce less milk, or are note able to produce milk at all. Some women with breast implants have also reported that breastfeeding is somewhat more painful after having breast implants.

In a study conducted on MemoryShape™ Breast Implants (the type of implant Dr. Ali uses), only four of the 44 breast augmentation patients who attempted to breast feed following breast implant surgery experienced difficulty. This means that 91% of the women in the study who attempted to breast feed after receiving breast implants had no difficulty.

The type of surgical technique used seems to have some effect on the likelihood of breastfeeding difficulties. For example, peri-areolar surgical approach (in which the surgeon uses an incision around the areola/nipple) has a slightly higher incidence of breastfeeding difficulties than other techniques.

Studies have also measured silicon levels in breast milk. Those studies did not indicate higher levels of silicon in breast milk from women with silicone gel breast implants when compared to women who did not have implants.

In fact, The Institute of Medicine (IOM) and The American College of Obstetricians and Gynecologists (ACOG) both have issued statements encouraging women with breast implants to breastfeed, stating that “Breastfeeding should be encouraged in all mothers when possible, including those with silicone breast implants.”

Decision Checklist for Getting Breast Implants

Breast Implants: Decision Checklist –

The surgical procedure for breast implants consists of choices you and your plastic surgeon need to make to ensure the best possible outcome and the highest degree of satisfaction with your new breasts.

In this article, Detroit area board certified plastic surgeon Doctor Ali has put together a “checklist” of the decisions you will need to make when deciding upon breast implantsurgery.

☐ Plastic Surgeon

Your surgical team – including your plastic surgeon, nurses, and anesthetist – is the most important decision you need to make.

Make sure to inquire about the number of breast implant surgeries your plastic surgeon has performed, as well as whether he or she is board certified.

Detroit area plastic surgeon Doctor Ali is double board certified, and has performed thousands of breast implants. In addition to being voted one of Hour Detroit’s “Top Docs” – many doctors seek out Doctor Ali when they need plastic surgery for themselves and their families.

☐ Type of Breast Implant

Silicone breast implants are filled with silicone gel, which feels remarkably like natural breast tissue. Saline breast implants are filled with sterile salt water. While they do provide a uniform shape, firmness and feel, they are less popular than silicone implants because they do not feel as “natural”.

☐ Size and Shape of Implant

In addition to the size that will look best on your body and achieve your appearance goals, the profile, or projection (height of the top of the breast implant from your chest) must be chosen.

☐ The location of the incisions

The location of the incisions made to insert the breast implants result in different recovery times, as well as varying amounts of scarring.  There are 3 standard types of breast implant incisions:

1: Transaxillary Breast Implant Incision, in which an incision is made through a canal placed in the natural fold of the armpit tissue;

2: Periareolar Breast Implant Incision, which involves making the incision around the circumference of the areola (the pink or brown skin around the nipple);

3: Inframammary Breast Implant Incision, made underneath the breast, in the fold where the breast and skin attach. – is not recommended for silicone gel implants such as MemoryShape.

☐ Where the implants will be placed in your breasts

Breast implants may be placed either:

1:  Subglandular – putting the breast implant directly behind the mammary gland, but in front of the muscle; or

2: Submuscular – breast implant is positioned completely behind the chest muscle wall.

Caring for Your Breast Implants

Breast Implants: Mammograms & Long Term Care –

Just as there are some things you should do to make sure your breasts stay healthy, there are also steps you should follow to care of your breast implants over time. Mammograms, breast exams, protecting your implants from certain types of damage, and regularly monitoring for breast implants for rupture are all very important.

Mammograms and Breast Implants

Whether or not a woman has breast implants, having a mammogram is the best way to detect breast cancer. However, there are some unique mammogram considerations for women with breast implants.

Breast implants can make it harder for the technician to perform a mammogram. Breast implants can also make it harder to identify breast cancer on a mammogram.

As every woman knows, the mammogram machine squeezes the breast to make it flat while taking a picture. The pressure from this squeezing could make your implant rupture or cause gel bleed.

Be sure to tell the mammogram technologist that you have breast implants before the procedure, so she can use special techniques to get the best possible views of your breast tissue, taking steps to ensure that your breast implants will not rupture due to the mammogram. Show your Device Identification Card (that you will receive after surgery) to the mammography technician.

Breast surgeons recommend having a mammogram before your breast implant surgery to establish a “baseline” for comparison in the future. Then women are encouraged to have another mammogram 6 months to 1 year after your implant surgery to establish another ”baseline” with the breast implant present.

After that women with breast implants should have a mammogram every 1 to 2 years, or as advised by their doctor, just as women without breast implants should do.

Other Breast Exams with Breast Implants

You should also perform self-breast exams once a month. After your period ends, is a good time to examine your breasts. Your doctor can show you how to do a self-breast exam, and teach you to tell the difference between your breast implant and breast tissue. If you see or feel that something has changed, talk to your doctor promptly.

It can sometimes be hard for you to feel changes in your breast when the implant is there, so it is important to have regular breast exams by a doctor, as well.

Preventing Breast Implant Rupture

Breast implant are quite durable, and are built to handle day to day bumps, hugs, and other everyday activities. Most surgeons indicate that you can even continue to participate in sports without restrictions. So it is not necessary to be overly concerned about rupture. However, physical trauma (e.g. an auto accident or fall), excessive repetitive wear and tear, can lead to a rupture in some cases.

A medical condition called capsular contracture can also cause rupture in a very small percentage of breast implants. Massaging your breasts regularly can help keep the surrounding soft and malleable, and prevent capsular contracture. Always alert your plastic surgeon if you notice the formation of any hardened scar tissue so that he can intervene before a rupture occurs.

If you or your doctor suspect a rupture or “gel bleed” of your breast implants, an MRI should be taken as soon as possible. An MRI is the best way to detect breast implant ruptures or leaks.

Breast Implant Revision Surgery

Breast Implant Revision Surgery:
Correcting Problems with Secondary Breast Augmentation

Breast implant surgery is a safe and common procedure with rare complications when performed by an experienced board certified plastic surgeon like Dr. Ali. However, breast implant “revision” surgery is occasionally required – typically as a result of older or outdated breast implant technologies, or because a less qualified surgeon did not achieve the best results.

Keep in mind that breast implant revision surgery is more complex and involved than original or “primary” breast implant surgery.

While it is always essential to select an experienced board certified plastic surgeon like Dr. Ali for breast implant surgery, it is even more crucial to have revision surgery done by a highly qualified plastic surgeon. Dr. Ali has considerable experience with the correction of breast augmentation problems, and will offer you a consultation to discuss how he can improve your results from a previous surgery.

Reasons for Breast Implant Revision Surgery

The six most common reasons for breast implant revision surgery that we see in our Detroit area surgical practice are:

#1. Changing breast implant size

Some women may feel that they went “too big” when they were younger, and now desire a less exaggerated figure. Others may have gained weight, and wish to increase their breast size for a more proportionate look.

Breast revision surgery strictly for size change must be carefully considered, to ensure that the patient is a suitable candidate for a second surgery.

#2. Correcting capsular contracture

Capsular contracture occurs when the body forms thick scar tissue around the breast implant(s) – causing shape change, discomfort, increased firmness and pain.

Revision surgery may be required to release the scar tissue in order to alleviate the pain and give the breast a softer feel and more natural shape.

#3. Problems with breast implant position

Inexperienced doctors sometimes do not have the expertise to naturally place the breast implants.

Breast implant asymmetry – with one implant higher than the other – is a common error. Additionally implants may be located too far apart. This results in an absence of cleavage, or even in the breasts falling into the armpits when laying down.

In the opposite situation “uniboob” or “breadloafing” can occur when the breasts are positioned too high and/or too close together on the chest.

Bottoming out – where implants are positioned too low on the chest in relation to the nipple position – is another problem that may require revision surgery.

#4. Problems with the patient’s breast tissue

Thinning of the breast tissue – resulting from aging, weight loss, pregnancy, or breastfeeding – may result in the implants becoming more visible and the appearance less natural.

An elongation of the skin and sagging of the breasts can also occur over time as tissue elasticity is lost as a result of aging, sun damage or smoking.

#5. Problems with the implants themselves

Deflation or rupture of a breast implant requires immediate revision surgery – ideally within a few weeks to keep the pocket from shrinking.

Ruptured saline implants are quite obvious because the augmentation is rapidly and completely lost over a day or two. Ruptured silicone implants may be less obvious and may require an ultrasound or MRI.

However, all breast implants used today have a full replacement warranty that will provide you with replacement implant(s) at no cost to you.

Breast Massage after Breast Implant Surgery

FAQs: Massaging Breasts after Breast Implant Surgery –

Massaging your breasts after breast implant surgery has many benefits. However, when and how you should massage your breasts after implant surgery will depend upon a variety of factors – including the type of implants and the surgical technique used.

In this article we explain the “basics” of breast massage after implant surgery. However, Dr. Ali will review the specific massage techniques that are right for you during your first or second post-operative visit. You should not begin massage until instructed to do so by your surgeon.

Breast Implant Massage FAQs

Why do I need to massage my breasts after augmentation?

Massage your breast implants after your surgery keeps the soft tissue “pocket” around the breast implant open, soft and supple.

The goal of the massage is to keep the pocket and soft tissue “capsule” around the breast implant a little bigger than the implants. This allows the implants to keep moving freely and naturally with the breast.

Massage helps to keep the pocket while the muscle and skin are still relaxing and the implants are “settling”. This helps limit the risk of “capsular contracture” in which the soft tissue capsule shrinks and squeezes the implant.

When do I massage my breasts after breast augmentation?

Typically, patients are advised to perform implant self-massage twice daily, beginning about one week after breast implant surgery.

Dr. Ali will examine your tissue and incisions before instructing you to start massaging the breasts.

How do I massage my breasts after breast implant surgery?

You should be lying down when doing breast massage, so that the overlying muscles will be relaxed, and the implants will move around more easily.

Typically, you will be instructed to push down toward your ribs on the edge of the implant – displacing it. You will generally be instructed to hold the implant in a displaced position for about 15 seconds – and repeat this five times in all directions.

However, in some cases, Dr. Ali may recommend a certain pattern of massage based on your specific case and needs.

How long do I need to massage my breast implants?

Your surgeon will give you detailed instructions – but typically you should massage your implants until they have fully settled – usually for about three months.

However, it is not a bad idea to continue the implant massages, at least two or three times a week, for as long as you have the breast implants.
