One area some patients are sometimers apprehensive about is the idea of “going under” with general anesthesia. In fact, recent studies report that nearly one in three people worry about the anesthesia more than the surgical procedure itself. Thanks to modern pharmaceuticals, and the the latest technologies in anesthetic administration, the risk of general anesthetic complications has decreased more than 90% in the last 50 years.

Complications with anesthesia dropped more than half from 2010-2013. And mortality rates from anesthesia remain inceredibly low, at only 0.03% of all surgeries. And it is important to remember that these numbers all take into account people who were in poor health to begin with, as well as those who’d had surgeries done in emergency situations.

How to Prepare for General Anesthesia in Breast Augmentation Surgery

Properly preparing for your breast augmentation surgery –and general anesthesia—helps to avoid unnecessary complications.

Be sure to tell your doctor all medications you’re taking, including herbal supplements or remedies. Many of these can interact with anesthesia medications, so it’s crucial that you’re thorough and completely honest with your doctor. You also need to fully disclose any medical conditions or procedures you may now have. or have had in the past.

If you take any ongoing medications, including blood thinners and those for diabetes or blood pressure, Dr. Ali will discuss any changes you may need to make the day of the surgery, as well as in the weeks before.

You will be given specific pre-surgery instructions, which you should follow carefully. Generaly you will be instructed to fast for six hours or more before the surgery to make sure the anesthesia has the desired effect.

Finally, be sure to arrange a ride home for yourself. You will need someone to drive you back after your surgery, so this should be figured out beforehand.

Board certified Breast Augmentation Surgeon Doctor Ali will decide which type of anesthesia is right for you, based on your current and past health, relevant test results, and the type of Breast Augmentation surgery being performed. He will discuss all of this with you during your consultation. The Day of Breast Augmentation Surgery

You will meet your anesthesiologist on the day of your Breast Augmentation surgery. He or she will constantly monitor your vital signs (heart rate, breathing, blood pressure) before, during, and after the surgery – and will also continuously adjust the levels of anesthesia you’re given. If there are any issues related to anesthetic during the Breast Augmentation surgery, they will correct them by giving additional fluids or medications.

Your anesthesia medications may be given intravenously (IV) or inhaled through a mask or both.

You will be free of pain, consciousness, movement, and memory during the time you’re under general anesthesia. Many people describe their experience as a “time warp,” where they aren’t sure how much time has passed when they wake up after surgery.

How quickly you wake up afterward depends on multiple factors: the type of anesthetics and other medications used, how you responded to them, and what amounts were given.

After your Breast Augmentation surgery, you will go to a recovery room where you’ll continue to be monitored. Your vital signs and bandages will be checked, and you’ll be asked how you’re feeling. If you experience any pain, be sure to tell your medical team so they can help you.

Side Effects of General Anesthesia After Breast Augmentation Surgery

There may be some side effects of anesthesia. These are temporary, but can last for several hours after surgery.

Some common effects of General Anesthesia after Breast Augmentation Surgery are:

  • Weakness or changes in muscle control
  • Altered coordination

  • Sleepiness

How Breast Surgeons Correct Inverted Nipples

What Is an Inverted Breast Nipple?

An inverted nipple is a nipple that protrudes inward, instead of outward. It can look flat, mishappen, or even appear to be a sunken hole where the nipple would normally be located.

Nipple inversion is more common than most people realize, affecting at least 2% of all men and women. Some people are born with inverted nipples, while others have nipples that become inverted later in life due to breastfeeding, a tumor pulling the nipple inward, trauma or other conditions.

An inverted nipple can cause embarrasment during intimacy, in the locker room or fitting room, or any time a woman’s breast may be seen. In addition to cosmetic concerns, inverted nipples can also sometimes cause functional problems, such as making breastfeeding difficult.

How Breast Surgeons Correct Inverted Nipples

Breast surgeons can perform a simple and routine surgery on the nipples that restores a natural appearance. There is usually no to very little scarring, and the milk ducts are not affected for future breastfeeding.

What to Expect After Nipple Correction Surgery

You will feel some discomfort for a few days after surgery, but this is easily managed with pain medication. After the Nipple Correction Surgery, some bruising and swelling is normal, but this typically only lasts for a few days.

You will be encouraged to begin walking around on the day of surgery, and can return to work as soon as you feel well enough. Many patients return to work as early as a day or two after surgery. Any stitches will generally be removed about 1 to 2 weeks following the procedure.

Patients are advised to avoid strenuous activity as well as sexual activity for a week or two. You should also avoid exposing the resulting small scars to the sun for at least six months. Sunlight can permanently affect the skin’s pigmentation, causing the scar to turn dark. If sun exposure is unavoidable, use a strong sun block.

No Breast Implants for Women who are Pregnant or Nursing

While it seems like “common sense” it is important to note that breast implant surgery should not be performed on a woman who is pregnant or nursing. However, women may get breast augmentation surgery before or after becoming pregnant and/or nursing.

Always ensure you are not pregnant before undergoing breast implants, taking a pregnancy test if there is any doubt or possibility that you may be carrying a child.

Breast Implants Before Becoming Pregnant

Some patients have asked us if having silicone gel breast implants could harm babies whose mothers became pregnant after getting breast augmentation.

We understand that nothing is more important to you than the health of your children! There is no evidence to shows that silicone breast implants have any harmful effects on children born to women with implants.

Two clinical studies in humans have found that there is NO increase in the risk of birth defects in children conceived and born after a women has had breast implant surgery.

Breast Implants Effect on Breast Feeding

In some women, there is a very small possibility that breast implant surgery might interfere with the ability to breastfeed. A very small percentage of women who have had breast augmentation surgery produce less milk, or are note able to produce milk at all. Some women with breast implants have also reported that breastfeeding is somewhat more painful after having breast implants.

In a study conducted on MemoryShape™ Breast Implants (the type of implant Dr. Ali uses), only four of the 44 breast augmentation patients who attempted to breast feed following breast implant surgery experienced difficulty. This means that 91% of the women in the study who attempted to breast feed after receiving breast implants had no difficulty.

The type of surgical technique used seems to have some effect on the likelihood of breastfeeding difficulties. For example, peri-areolar surgical approach (in which the surgeon uses an incision around the areola/nipple) has a slightly higher incidence of breastfeeding difficulties than other techniques.

Studies have also measured silicon levels in breast milk. Those studies did not indicate higher levels of silicon in breast milk from women with silicone gel breast implants when compared to women who did not have implants.

In fact, The Institute of Medicine (IOM) and The American College of Obstetricians and Gynecologists (ACOG) both have issued statements encouraging women with breast implants to breastfeed, stating that “Breastfeeding should be encouraged in all mothers when possible, including those with silicone breast implants.”

The location of the incisions made to insert the breast implants result in different recovery times, as well as varying amounts of scarring.  There are 3 standard types of breast implant incisions:

1: Transaxillary Breast Implant Incision, in which an incision is made through a canal placed in the natural fold of the armpit tissue;

2: Periareolar Breast Implant Incision, which involves making the incision around the circumference of the areola (the pink or brown skin around the nipple);

3: Inframammary Breast Implant Incision, made underneath the breast, in the fold where the breast and skin attach. – is not recommended for silicone gel implants such as MemoryShape.

Caring for Your Breast Implants

Breast Implants: Mammograms & Long Term Care – 

Just as there are some things you should do to make sure your breasts stay healthy, there are also steps you should follow to care of your breast implants over time. Mammograms, breast exams, protecting your implants from certain types of damage, and regularly monitoring for breast implants for rupture are all very important.

Mammograms and Breast Implants

Whether or not a woman has breast implants, having a mammogram is the best way to detect breast cancer. However, there are some unique mammogram considerations for women with breast implants.

Breast implants can make it harder for the technician to perform a mammogram. Breast implants can also make it harder to identify breast cancer on a mammogram.

As every woman knows, the mammogram machine squeezes the breast to make it flat while taking a picture. The pressure from this squeezing could make your implant rupture or cause gel bleed.

Be sure to tell the mammogram technologist that you have breast implants before the procedure, so she can use special techniques to get the best possible views of your breast tissue, taking steps to ensure that your breast implants will not rupture due to the mammogram. Show your Device Identification Card (that you will receive after surgery) to the mammography technician.

Breast surgeons recommend having a mammogram before your breast implant surgery to establish a “baseline” for comparison in the future. Then women are encouraged to have another mammogram 6 months to 1 year after your implant surgery to establish another ”baseline” with the breast implant present.

After that women with breast implants should have a mammogram every 1 to 2 years, or as advised by their doctor, just as women without breast implants should do.

Other Breast Exams with Breast Implants

You should also perform self-breast exams once a month. After your period ends, is a good time to examine your breasts. Your doctor can show you how to do a self-breast exam, and teach you to tell the difference between your breast implant and breast tissue. If you see or feel that something has changed, talk to your doctor promptly.

It can sometimes be hard for you to feel changes in your breast when the implant is there, so it is important to have regular breast exams by a doctor, as well.

Preventing Breast Implant Rupture

Breast implant are quite durable, and are built to handle day to day bumps, hugs, and other everyday activities. Most surgeons indicate that you can even continue to participate in sports without restrictions. So it is not necessary to be overly concerned about rupture. However, physical trauma (e.g. an auto accident or fall), excessive repetitive wear and tear, can lead to a rupture in some cases.

A medical condition called capsular contracture can also cause rupture in a very small percentage of breast implants. Massaging your breasts regularly can help keep the surrounding soft and malleable, and prevent capsular contracture. Always alert your plastic surgeon if you notice the formation of any hardened scar tissue so that he can intervene before a rupture occurs.
