Preparing for your Tummy Tuck
If you are considering a tummy tuck, the first step is to schedule an appointment. Dr. Ali is a board certified plastic surgeon in Macomb County, a member of the American Society of Plastic Surgeons, and a Fellow in the American College of Surgeons, with extensive experience performing cosmetic and reconstructive surgeries. When you meet with Dr. Ali, you should be prepared to discuss why you want a tummy tuck, and what you hope the results will be.
Dr. Ali will review your medical history, including any current or past medical conditions, what medications you are taking, past surgeries, and allergies to medications. He will also ask you detailed questions about your history of weight gain and loss. He will then examine your abdomen, to determine your treatment options.
You will also be informed about potential risks, including the potential for scarring. Dr. Ali will tell you to avoid certain medications (such as aspirin, anti-inflammatory drugs and some herbal supplements, which can increase bleeding), and if you are a smoker, you will need to stop at least several weeks prior to the surgery date.
Ideally, you will have maintained a stable weight for the year prior to getting your surgery. If you are currently overweight, Dr. Ali may recommend that you lose weight before scheduling the procedure. If you lose a significant amount of weight after the surgery is completed, it can lessen the desired results.
Once your tummy tuck is scheduled, you should arrange for help after the surgery and during your recovery. You will need someone to drive you home after you leave the hospital, and ideally, to stay with you at least the first night of your recovery at home.
Types of Tummy Tuck Surgery
There are generally three types of tummy tuck surgeries, depending whether loose skin, loose muscles, or both with need to be surgically tightened.
Mini Tummy Tuck
A “mini” tummy tuck improves the lower belly with less extensive surgery. A candidate for a mini may be bothered by a “pooch” or loose skin and stretch marks below the belly button. This technique involves a single incision made just about the pubic mound, through which the loose muscles are tightened. Excess skin will be removed to restore a flat, smooth abdominal wall. The resulting scar will be a thin, horizontal line, just a few inches long.
Classic or Full Tummy Tuck
A classic or “full” tummy tuck improves both the upper and lower abdomen. If you are concerned about roundness and/or excess skin folds above and below your belly button, the full tummy tuck may be for you. Here, either a horizontal or U-shaped incision is made above the pubic mound. Excess skin is then removed, and abdominal muscles are tightened. In some cases, a second incision is made around the navel to remove excess skin above the belly button. The scar will typically be a bit longer, running from hip bone to hip bone, but it can generally be hidden beneath a swimsuit bottom. A full tummy tuck will yield a flatter and smoother abdomen than the mini.
Extended Tummy Tuck
An extended tummy tuck is the most comprehensive procedure. If you have lost a lot of weight or have experienced a number of pregnancies and now have excess sagging skin hanging around the front half of your body, the extended tummy tuck may be something to consider. In addition to tightening the abdominal muscles and removing excess skin from the upper and lower abdomen, it also addresses excess skin around the flanks.

What You Can Expect During the Tummy Tuck Procedure
Tummy tucks are done in a hospital or an outpatient surgical facility. You will be under a general anesthesia, which means you will be completely unconscious and unable to feel any pain.
During the procedure itself, Dr. Ali will make incisions to remove excess skin between your belly button and pubic hair. Depending on the extent of your surgery, Dr. Ali might also address any excess fat, removing it the same way it would be removed during traditional liposuction.
The fascia, which overlies the abdominal muscles, will be tightened with permanent sutures. Finally, the incision will be stitched together.
The procedure typically takes about three hours from start to finish, but of course that depends upon what type of tummy tuck you choose to have.
Before finishing the procedure, Dr. Ali will probably place drains in your stomach to remove any extra fluid that may build up as you heal, allowing the newly stretched skin to stick to the muscle. (The drains will be removed after about a week.)
What Happens to the Belly Button in a Tummy Tuck
In a full tummy tuck, the skin above the belly button will be pulled down so that it will cover both the upper and lower abdomen. Some patients are surprised to notice that an existing birthmark or mole that was located above their belly button before the operation has moved to below the belly button after the surgery. Your belly button, however, will be brought out through a small incision and sutured in its normal position.
After a Tummy Tuck
After spending time in recovery, you will be released to go home, and your recovery process will begin. Before you leave, Dr. Ali will likely cover your abdominal incision and your bellybutton with surgical dressings. Small tubes may be placed along the incision to drain any excess fluid or blood. You will need to have a driver take you home, and it is a good idea to have arranged for someone to be with you for at least the first night after the surgery.
The length of your recovery will depend upon the type of tummy tuck performed, as well as how much skin was removed. The recovery period is usually much shorter for a mini-tummy tuck.
Drains may be left in place at the surgery site for a few days after the surgery. Typically, DR. Ali will also prescribe antibiotics and an anticoagulants while the drains are in place. You will also be given a prescription for pain management – although some patients only require over-the-counter pain relievers.
Recovery from a Tummy Tuck
Once you get home, the recuperation process begins. Most patients find that positioning their bed to keep the upper body slightly elevated and knees at an angle for the first few days after surgery is helpful in terms of comfort level.
It could take a few days to a week before you feel completely comfortable standing fully erect. Patients often walk slightly bent over due to the tightness of their abdomens.
Light walking right after the surgery is advised, and in fact, it’s good for you. However, you should avoid hitting the gym for at least two weeks, and consult with Dr. Ali before you try anything strenuous. Generally, tummy tuck patients take at least two weeks off of work, and maybe more, depending on what the job entails. After about five weeks, most patients are usually able to completely resume their normal activities. Of course, everyone is different.
Patients typically wear an abdominal binder for about six weeks, to avoid fluid buildup and help support the abdomen. Patients need to avoid strenuous activity, including exercise and lifting for at least six weeks.

Ease Back into Exercise after Getting Breast Implants
Even if you feel great – take things slowly as you return to exercise and other strenuous activities. Avoid heavy lifting and strenuous exercise for at least a month post-surgery. While you can resume mild exercise about a week after surgery (follow your doctor’s instruction) you’ll want to take things very slowly.
Start simply and work up – walking before running; and replacing high impact activities with low impact exercises. Avoid heavy lifting, including weights or weight machines, until your doctor specifically tells you it is “safe” and you are not in danger of tearing stitches.
Wear Proper Breast Support after Getting Breast Implants
Gravity still affects breast implants – though not to the same extent as it affects natural breast tissue. So if you have got implants, you need a great, supportive bra.
Remember, while the implants themselves will always keep their shape, your skin, muscles and tissue must support the weight of the breast implants. So you need to limit the impact that gravity has on your body to prevent premature sagging of your breasts – especially when exercising or running!
Most breast implant surgeons recommend that you wear a bra as often much as you can – even when sleeping at night, to protect the breasts from excessive bouncing and movement.

Protecting Your Breast Implants After Reconstruction

If you have survived breast cancer and a mastectomy, and have undergone breast reconstruction surgery, you have already been through so much! You are a warrior! So, of course, you want to do everything possible to protect your new breasts – and ensure that you do not have to undergo any further breast revision surgery.
In December, a new clinical study published in the Journal of the American College of Surgeons reported that two huge risk factors – smoking and obesity – can dramatically increase the odds of early implant loss after breast reconstruction with implants following a mastectomy.
The study was conducted at the Perelman School of Medicine at the Hospital of the University of Pennsylvania, Philadelphia.The stated goal of the study was figure out which patients are at greatest risk for complications within the first 30 days after breast reconstruction.
In order to determine which risk factors were associated with 30-day breast implant loss for any reason data from the American College of Surgeons National Surgical Quality Improvement Program was analyzed. The study included more than 14,500 women between the ages of 40 and 60 who had undergone immediate breast reconstruction with implants after a mastectomy.
The results of the study were starling:
Smoking was shown to increase woman’s risk of early implant loss by three times!
And obesity as shown to increase woman’s risk of early implant loss by three times! Additionally, the more severe the state of obesity, the higher the patient’s risk of losing the implant was.
Other Risk Factors for Breast Implant Loss
Other lesser factors that were associated with a greater chance of complications included age, with women over age 55 being at slightly greater risk for implant failure. Additionally, direct-to-implant reconstruction – undergoing mastectomy and implants in a single procedure – also increased the risk of implant failure within the first 30 days after breast reconstruction surgery.
Less than one percent of all patients in this study of 14,583 women (so about 14) experienced complication involving implant loss in 30 days or less. Patients in to lower risk groups only had a, failure rate of .39% . But patients in the high risk group – due to smoking or obesity – had a failure rate of 3.86%.
This is actually quite a substantial difference – because it means that one in 25 patients in the high-risk group will lose a breast implant within the first 30 days.
The takeaway from this is that while smoking and obesity have a variety of negative health consequences, their impact on breast reconstruction surgery can be dire. We understand that losing weight or quitting smoking is a difficult uphill battle. But in order to ensure that your reconstruction surgery is a success, we urge you to find the support that you need so that you can give up cigarettes or lose some of the extra weight.

From high to low risk, the type of materials to consider include: Round Textured Saline implants which have the greatest risk of rippling, Teardrop Textured Saline with slightly less risk, Smooth Saline implants, then Textured Silicone and finally, Smooth Silicone implants that have minimal risk of rippling.

Breast Implant Movement
After undergoing breast implant surgery, there are numerous things to consider regarding breast movement. One factor will be the type of breast implant you’ve selected, as will be the natural shape and size of your breasts. As they are soft and fluid, the majority of breast implants move naturally. However, on smaller breast implants or on breast implants underneath tighter skin, in most cases the implants remain slightly firmer and move less than larger breasts do.
Breast Implant Position
For a brief period of time after your breast implant surgery, your breasts will most likely rest higher on your chest. After several weeks or so, will drop down to their desired position. The exact time it will take for your breast implants to drop depends on where your implants are placed, their size, your muscle tone, the surface type of your implants, and the size of your breasts before your surgery. Smoother breast implants drop more rapidly than textured breast implants, and regular massaging of your breasts is an excellent way to speed up the process after surgery!
One major factor influencing the speed with which your breast implants drop is their placement. Breast implants placed under the muscle will take longer to drop than breast implants placed above the muscle.
Also, if you began with smaller breasts and opted to significantly increase your size, the tightness of your skin may result in it taking slightly longer for your breasts to drop. You may even experience one breast dropping prior to the other! While this varies from case to case, don’t worry if one breast drops before the other.
Following your breast augmentation surgery, you can speed up the dropping process by massaging your breasts. Additionally, you can help push down your breast implants by wearing a wide elastic band around the top of your chest. At the absolute longest, it can take about six to nine months for your breast implants to drop, but most breast implants drop within a period of several weeks.
Breast Implant Appearance
After undergoing breast implant surgery, it is perfectly normal for your breasts to appear slightly shiny or for you skin to experience some redness. Since your skin has been stretched out to allow room for your breast implants, you may experience a sensation comparable to a mild sunburn. This is perfectly normal, and is simply an element of the healing process. In addition to slight redness, your skin may take on a slightly shiny appearance following your breast implant surgery. This results from your skin being stretched, and as you skin continues to heal, the shininess will fade.
Breast Implant Softness
There are a number of factors that play into how long it takes for your breast implants to soften up. This includes your type of implant filler, the surface type of your breast implant, your breast implant’s placement, the tone of your muscle, and the fullness of your breast implant.
In the brief period immediately following your breast implant surgery, your implants will remain relatively firm. Generally, saline implants are firmer than silicone implants, and textured implants feel firmer than smoother ones.
