How Breast Reduction Surgery is Performed
Breast reduction surgery is performed under general anesthesia – which means that the patient is “put to sleep” and not conscious during the surgery. Dr. Ali typically uses one of two different incision types, depending on the patients breast size and their post-surgery appearance goals.
The first is a three-part incision technique, known as an anchor incision. The incisions are made around the areola, first vertically to the crease beneath the breast and then horizontally beneath the breast, paying close attention to follow the natural crease of the breast.
The other incision type is referred to as the lollypop technique. In this surgical technique, Dr. Ali will take great care to limit the incision to around the nipple and straight down the fold of the breast, eliminating the need for the horizontal incision along the crease of the breast.
Dr. Ali will remove the desired amount of excess fat and tissue, and carefully reposition the nipple and areola. He can also reduce the size of the areola to fit better with the new breast size. There will be discomfort for a few days, and you will be given medication to control any post operative pain. Bruising and swelling will begin to go away over the weeks following surgery. Jessica’s Breast Augmentation Story
As a mother of three teenagers who all go to Birmingham Groves High School, I am always super busy. And, like most moms, I’ve pretty much always put my family’s needs before my own. Also, like many mothers, I struggled with the physical toll that motherhood can take on the body, particularly on my breasts.
After three pregnancies, I never really liked the appearance of my breasts. But I also never found time to do anything about it. The weight gain of pregnancy plus breastfeeding 3 kids really took its toll, and my breasts were saggy with the nipples pointing downward. Despite my efforts to try to maintain a healthy lifestyle, my breasts were left drooping and flat.
It wasn’t until my kids were in high school that I finally found the time and motivation to prioritize my own health and my self-esteem. I joined a gym, cut out snacks and junk food, and finally shed the “baby weight” I had gained over the years. With a lot of determination, I successfully lost 40 pounds and felt really proud of my accomplishment.

Ashley’s Breast Augmentation Story
Growing up in Troy, MI, I always felt I had a body that didn’t quite fit what girls and women were supposed to look like. As a teenager, I felt self-conscious and insecure about my small and uneven breasts. It was a source of embarrassment that followed me from high school through adulthood. While my friends seemed to blossom into women, I remained stuck in a state of constant dissatisfaction with my appearance.

The scarring “outlines” the areola, blending-in with the natural change in flesh pigment. And in the unlikely event of a complication or additional surgery, the implant can be inserted, removed, or adjusted through the same incision.
The advantage of peri-areolar breast implant incision is that it allows for precise placement of the implant while minimizing visible scarring, as the incision blends with the natural color transition of the areola. This type pf breast implant incision also provides access to the breast pocket for both subglandular and submuscular placement of implants. And it can be combined with areolar (nipple) resizing for improved symmetry and aesthetics.
2. Inframammary Fold Breast Implant Incision
The inframammary fold breast implant incision is made in the crease under the breast. This allows the fullness of the breast to cover any scarring. It is ideal for both saline and silicone gel implants, as well as for primary breast augmentation, revision surgeries, and breast lifts. It is a very common incision used for breast implants, and can be used for sub-glandular, sub-pectoral, or sub-muscular placement of the implant.
The advantage of the inframammary fold breast implant incision is that it provides direct access to the breast pocket, allowing for precise implant placement and adjustment. And, because the incision is concealed within the natural crease of the breast, it minimizes visible scarring.
When increasing breast size considerably, however, the plastic surgeon typically must create a new crease so that the nipple is centered on the new, larger breast. 3. Transaxillary Breast Implant Incision

Once the incision is made in the fold of the armpit, a channel is cut going down to the breast (see image below). The breast implant is inserted into the channel and worked downwards into position. Sometimes an endoscope (a small surgical camera on the end of a tube) will be used by the cosmetic surgeon during this procedure.
However, positioning the breast properly from farther away makes placement more difficult. For this reason a skilled and experienced board certified plastic surgeon like Dr. Ali should be chosen, to ensure proper positioning of the breast.
The advantages of transaxillary breast implant incisions are the minimizing of visible scarring on the breast itself, as the incision is located in the armpit. This incision also avoids interference with breast tissue, reducing the risk of affecting breastfeeding and nipple sensation.
Transaxillary breast implant incisions primarily used for saline implants placed behind the chest muscle (submuscular), particularly in patients who prefer to avoid breast incisions or who have certain specific anatomical considerations.
The disadvantage to a transaxillary breast implant incision is that if a complication occurs requiring revision or removal, the plastic surgeon will typically have to make a new nipple or crease incision. This is because it’s very rare that a transaxillary incision can be reused.
