What Is a Tummy Tuck?

A “tummy tuck” tightens and reshapes the abdominal muscles and removes excess skin, giving your abdominal area a more pleasing, slimmer overall appearance. As a bonus, a tummy tuck can also take care of excess fat. However, the primary focus for the procedure focuses on the skin and the muscle.

During a tummy tuck, the abdominal muscles are tightened, and excess sagging skin is removed. This leaves you with a flatter and better toned midsection, and corrects a protruding belly.

The skin is rather like elastic. Once it becomes too loose, it doesn’t spring back like it used to, leaving you with loose, sagging skin. No matter how much you exercise, and no matter how much weight you lose, that loose skin will not become taut again.

And then there’s the muscles. Normally, there is a little separation of perhaps a half inch or so between both sides of your “six pack” (technically known as the rectus muscle). As you age, due to certain lifestyle factors (pregnancy, lack of exercise, or weight gain/loss), the separation grows larger, and it loses its ability to “hold you in.” Unfortunately, once these muscles have spread apart, surgery is the only way to tighten them. A tummy tuck acts like an internal corset, and brings the muscle edges together, allowing them to pull your belly in.

Candidates for a Tummy Tuck

The decision to have any cosmetic procedure is very personal, and there are many reasons people decide to have tummy tucks. Some have had success losing weight with diet and exercise, but their abdomens still lack tone and protrude. Others wish to get rid of the excess skin that hangs around their belly after weight loss. Many women experience stretched out abdominal muscles and skin following pregnancy. And some people have bellies that “stick out” even though they are otherwise in good shape. A tummy tuck can help in any of these situations, resulting in a firmer, flatter abdomen.

Tummy tucks are popular for both men and women, especially those whose bodies have undergone significant changes from weight loss or, in the case of women, pregnancy. However, tummy tucks are not for everyone.

Remember, tummy tucks have nothing to do with fat. The primary focus is on muscle repair and skin removal. If excess fat is the major focus as opposed to muscle separation or loose skin, or for those who plan on having children, there may be other, better options. In fact, about half of the women who think a tummy tuck is the answer to a flatter belly end up having liposuction instead. In males, the percentage is even higher.

Reasons to Put Off Having a Tummy Tuck

If you are still planning on having children or if you want to lose a significant amount of weight, you may want to put off making a decision to have a tummy tuck. Either of these factors can negatively affect the end result. If you think you may want more children, or if you are still working on reaching your desired weight, it’s best to hold off on a tummy tuck until these milestones are reached.

Some Alternatives to a Tummy Tuck

If excess fat is your main concern, liposuction may be something to consider. Liposuction is a cosmetic procedure that removes the fat that you can’t seem to get rid of through diet or exercise. People who have liposuction generally have a stable body weight, but would like to remove undesirable body fat in specific parts of the body. However, it does not remove cellulite or stretch marks, nor does it address muscle separation or loose skin.

How Tummy Tuck Compares to Non-Invasive Body-Sculpting

Tummy tucks are considerably more involved in terms of the surgery itself, and of course, the recovery. Noninvasive body-contouring procedures such as CoolSculpting and SculpSure are surface level procedures, and therefore can’t come close to achieving the results of a tummy tuck. The way these procedures work is by either freezing or heating fat cells to kill them off, but like liposuction, they don’t address the sagging skin or underlying muscle separation the tummy tuck targets.

If your major concern is excess fat, these noninvasive options can usually reduce superficial fat by an average of 25 percent. So, if you still have tight elastic skin and toned muscles that aren’t stretched, you may find the non-invasive options to be reasonable.

Preparing for your Tummy Tuck

If you are considering a tummy tuck, the first step is to schedule an appointment. Dr. Ali is a board certified plastic surgeon in Macomb County, a member of the American Society of Plastic Surgeons, and a Fellow in the American College of Surgeons, with extensive experience performing cosmetic and reconstructive surgeries. When you meet with Dr. Ali, you should be prepared to discuss why you want a tummy tuck, and what you hope the results will be.

Dr. Ali will review your medical history, including any current or past medical conditions, what medications you are taking, past surgeries, and allergies to medications. He will also ask you detailed questions about your history of weight gain and loss. He will then examine your abdomen, to determine your treatment options.

You will also be informed about potential risks, including the potential for scarring. Dr. Ali will tell you to avoid certain medications (such as aspirin, anti-inflammatory drugs and some herbal supplements, which can increase bleeding), and if you are a smoker, you will need to stop at least several weeks prior to the surgery date.

Ideally, you will have maintained a stable weight for the year prior to getting your surgery. If you are currently overweight, Dr. Ali may recommend that you lose weight before scheduling the procedure. If you lose a significant amount of weight after the surgery is completed, it can lessen the desired results.

Once your tummy tuck is scheduled, you should arrange for help after the surgery and during your recovery. You will need someone to drive you home after you leave the hospital, and ideally, to stay with you at least the first night of your recovery at home.

Types of Tummy Tuck Surgery

There are generally three types of tummy tuck surgeries, depending whether loose skin, loose muscles, or both with need to be surgically tightened.

Mini Tummy Tuck

A “mini” tummy tuck improves the lower belly with less extensive surgery. A candidate for a mini may be bothered by a “pooch” or loose skin and stretch marks below the belly button. This technique involves a single incision made just about the pubic mound, through which the loose muscles are tightened. Excess skin will be removed to restore a flat, smooth abdominal wall. The resulting scar will be a thin, horizontal line, just a few inches long.

Classic or Full Tummy Tuck

A classic or “full” tummy tuck improves both the upper and lower abdomen. If you are concerned about roundness and/or excess skin folds above and below your belly button, the full tummy tuck may be for you. Here, either a horizontal or U-shaped incision is made above the pubic mound.  Excess skin is then removed, and abdominal muscles are tightened. In some cases, a second incision is made around the navel to remove excess skin above the belly button. The scar will typically be a bit longer, running from hip bone to hip bone, but it can generally be hidden beneath a swimsuit bottom. A full tummy tuck will yield a flatter and smoother abdomen than the mini.

Extended Tummy Tuck

An extended tummy tuck is the most comprehensive procedure. If you have lost a lot of weight or have experienced a number of pregnancies and now have excess sagging skin hanging around the front half of your body, the extended tummy tuck may be something to consider. In addition to tightening the abdominal muscles and removing excess skin from the upper and lower abdomen, it also addresses excess skin around the flanks.

What You Can Expect During the Tummy Tuck Procedure

Tummy tucks are done in a hospital or an outpatient surgical facility. You will be under a general anesthesia, which means you will be completely unconscious and unable to feel any pain.

During the procedure itself, Dr. Ali will make incisions to remove excess skin between your belly button and pubic hair. Depending on the extent of your surgery, Dr. Ali might also address any excess fat, removing it the same way it would be removed during traditional liposuction.

The fascia, which overlies the abdominal muscles, will be tightened with permanent sutures. Finally, the incision will be stitched together.

The procedure typically takes about three hours from start to finish, but of course that depends upon what type of tummy tuck you choose to have.

Before finishing the procedure, Dr. Ali will probably place drains in your stomach to remove any extra fluid that may build up as you heal, allowing the newly stretched skin to stick to the muscle. (The drains will be removed after about a week.)

What Happens to the Belly Button in a Tummy Tuck

In a full tummy tuck, the skin above the belly button will be pulled down so that it will cover both the upper and lower abdomen. Some patients are surprised to notice that an existing birthmark or mole that was located above their belly button before the operation has moved to below the belly button after the surgery. Your belly button, however, will be brought out through a small incision and sutured in its normal position.

After a Tummy Tuck

After spending time in recovery, you will be released to go home, and your recovery process will begin. Before you leave, Dr. Ali will likely cover your abdominal incision and your bellybutton with surgical dressings. Small tubes may be placed along the incision to drain any excess fluid or blood. You will need to have a driver take you home, and it is a good idea to have arranged for someone to be with you for at least the first night after the surgery.

The length of your recovery will depend upon the type of tummy tuck performed, as well as how much skin was removed. The recovery period is usually much shorter for a mini-tummy tuck.

Drains may be left in place at the surgery site for a few days after the surgery. Typically, DR. Ali will also prescribe antibiotics and an anticoagulants while the drains are in place. You will also be given a prescription for pain management – although some patients only require over-the-counter pain relievers.

Recovery from a Tummy Tuck

Once you get home, the recuperation process begins. Most patients find that positioning their bed to keep the upper body slightly elevated and knees at an angle for the first few days after surgery is helpful in terms of comfort level.

It could take a few days to a week before you feel completely comfortable standing fully erect. Patients often walk slightly bent over due to the tightness of their abdomens.

Light walking right after the surgery is advised, and in fact, it’s good for you. However, you should avoid hitting the gym for at least two weeks, and consult with Dr. Ali before you try anything strenuous. Generally, tummy tuck patients take at least two weeks off of work, and maybe more, depending on what the job entails. After about five weeks, most patients are usually able to completely resume their normal activities. Of course, everyone is different.

Patients typically wear an abdominal binder for about six weeks, to avoid fluid buildup and help support the abdomen. Patients need to avoid strenuous activity, including exercise and lifting for at least six weeks.

A “mini-tummy tuck” also removes excess skin and fat, but does not tighten muscles, and is used when the navel does not need to be repositioned.

Full Abdominoplasty is the most extensive type of tummy tuck surgery. It is appropriate for women with very lax muscles, as well as larger amounts of excess loose skin.

Tummy Tucks are On the Rise

New Jersey plastic surgery consultants, Associates in Plastic Surgery, reports that the number of tummy tucks performed from the years 2000 to 2015, increased 104%.

According to a 2014 study published in Plastic and Reconstructive Surgery®, the official medical journal of the American Society of Plastic Surgeons (ASPS), tummy tucks are one of the most common cosmetic surgical procedure performed in the United States, with more than 117,000 procedures performed that year alone.

The ASPS went on to state that 97% of all tummy tuck patients were female, with 65% of patients undergoing the surgery in conjunction with another cosmetic procedure.

Tummy Tucks after Pregnancy or Gastric Bypass

It is estimated that almost half (44 percent) of tummy tucks are done in women in their 20s and 30s. The most common reason for tummy tuck surgery in this age group is to treat diastasis recti a common post-pregnancy problem in which the abdominal muscles, separate – resulting in a sagging belly.

But women aren’t the only one’s getting flat abs with abdominoplasty. According to the American Society of Aesthetic Plastic Surgery, the number of cosmetic surgeries performed on men has increased more than 325% since 1997.

In fact tummy tucks were second only to liposuction for both men and women in the most commonly performed cosmetic surgery!

The second most common reason for undergoing a tummy tuck is skin tightening after a gastric bypass or other successful major weight loss. Almost all patients who undergo bariatric surgery end up with sagging skin at the end of their weight loss journey. In some cases, insurance will even cover all or part of loose skin removal after bariatric surgery – if it can be shown that it is  for “symptom relief” rather than strictly cosmetic.

Tummy Tuck vs. CoolSculpting: Anesthetic

CoolSculpting is a noninvasive procedure, performed without anesthetic – while a Tummy Tuck is a more serious surgical procedure typically performed under general (unconscious) anesthetic.

CoolSculpting is a noninvasive procedure that freezes and removes excess fat deposits. In a CoolSculpting procedure, there is no removal of skin. As such, once the fat is removed, the skin must tighten on its own. And unlike abdominoplasty, the CoolSculpting procedure does not tighten the abdominal muscles.

Tummy Tuck vs. CoolSculpting: Scarring

Typically, a full tummy tuck requires an incision across the lower abdomen. This allows the plastic surgeon to tighten the muscles and remove excess skin. As with any incision, this results in minor scarring.

Because CoolSculpting is nonsurgical, there is no scarring at all.

Endoscopic tummy tucks, (“mini tummy tucks”), result in significantly less scarring than a full tummy tuck – but, as they are still surgical procedures, result in more scarring than the scar-free CoolSculpting.
