Breast Implant Recovery Calendar

Days 1 to 4 after Breast Implant Surgery

This is the inflammatory period, where breasts are swollen, and may feel tight, uncomfortable or even be painful. During this period most patients require some medication to be comfortable. Bruising may start to appear, and fluid retention is also common.

Days 5 to 10 after Breast Implant Surgery

If permitted by your surgeon, it is now generally OK to shower and get the incisions briefly wet.

Pain now tends to occur more at night – typically between 3a.m. to 6 a.m. Usually you will be instructed by your breast implant surgeon to begin breast massages during this period.

Your external sutures will be removed – and/or if the surgeon used tissue glue or tape it will begin to fall off after a week or two. After a week you may even be able to return to work – if approved by your surgeon.

Be aware of signs of bleeding or infection during this time period and alert your surgeon if anything seems amiss.

Days 11 to 21 after Breast Implant Surgery

By now there is very low risk of infection and bleeding. You can typically now increased physical activity, such as low-impact exercises.

The majority of the swelling should begin to subside, however you may still have some occasional pain at night.

In some individuals ‘pins and needles’ sensations in the nipple area may be experienced as the nerves begin to “wake up”. This, along with some areas of skin numbness, is normal – and will gradually subside.

Days 22 to 42 after Breast Implant Surgery

By now you should not require any pain medications – although you can generally take ibuprofen or Tylenol, with your surgeon’s permission. You should be able to transition to higher impact aerobic activities, during this time period, as well.

Day 43 to 9 Months after Breast Implant Surgery

The scar tissue should be relaxing, softening the initial firm and tight feeling of the breast implants. The remaining 5 to 10% of swelling should also subside.

At this point most women will have a good idea of what the final results look like. Although, some women may feel faster or slower than the “norm”.

Saline Breast Implants

Saline breast implants are filled with sterile salt water. They do provide a uniform shape, firmness and feel, but are less popular than silicone implants because they do not feel as “natural”.

Structured saline breast implants contain an inner structure which helps make the breast implant feel more natural.

If the saline filled breast implant shell leaks, it will collapse and the saline will be absorbed and naturally expelled by the body. Saline breast implants also offer the advantage of requiring a smaller incision.

Silicone Breast Implants

Silicone breast implants are filled with silicone gel, which feels remarkably like natural breast tissue. Silicone breast implants are FDA-approved for breast augmentation in women age 22 or older.

Some women are concerned about silicone breast leaks because of inferior breast implant technologies that were in use many years ago. However this should not be a concern. Today’s silicone breast implants are far superior, and in rare cases where a leak occurs, the gel may remain within the implant shell. This means that a leaking implant filled with silicone gel will not collapse. And, all breast implants manufactured for use in the United States come with a lifetime manufacturer’s warranty.

“Gummy bear breast implants” contain a silicone gel inside the implant that is thicker than traditional silicone gel implants. They maintain their shape even if the implant shell is broken. How do I know if I should have my implants placed over or under the muscle?

Generally, you should choose submuscular (under the muscle) breast implant placement if you have saline implants, a smaller amount of natural breast tissue, or a strong family history of breast cancer.

Typically, you should choose subglandular (over the muscle) breast implant placement if you want silicone implants, have a larger amount of natural tissue, or require a breast lift.

How do I know if I need a breast lift?

Dr. Ali will offer you a consultation to determine if you will also need a breast lift at the time of your implants. However, one way to get an idea if you need a breast lift is to stand in front of a mirror and look at your nipples in relation to the natural crease beneath your breasts.

If your nipples sit lower than your breast crease, adding breast implants without a breast lift to remove excess skin will generally make them more saggy – so you will probably need a breast lift.

Should I get round or teardrop-shaped implants?

Dr. Ali will offer you a consultation to help you determine which shape of breast implants are best for you. However, generally, round implants are the most commonly used implants. Round implants are totally symmetric so that their “orientation” (placement location) is always even and symmetric. Round implants are also less expensive than teardrop implants.

Anatomic or teardrop implants, on the other hand, are not symmetrical, with a greater projection at the bottom of the implants than at the top. So in some women, they may have a tendency to rotate causing problems.

Additionally, keep their rounded shape making them look somewhat more natural when you are lying down.

Will I get more cleavage from a breast implants?

Yes, breast implants can give you more cleavage. However, it’s important to that increasing the size of your breasts is not what gives you cleavage. Cleavage is a result of the width of your chest and how close your breasts are together – not how big they are.

If your breasts are set wide on your chest, you will have more fullness after your implants, but not more cleavage. On the other hand, if your body is narrow below your shoulders and your breast implants are placed close together, you can achieve greater cleavage even with a small implant.

What cup size should I choose?

Women tend to think of breasts in terms of cup sizes, because that is how we buy our bras. But cup size is a very subjective measurement: everyone wears their bras differently and every bra manufacturer makes them differently. A woman may be a 36C in one bra and a 34D in another.

Breast implant sizes are actually measured in cc’s. Implants range from 120 to 960 cc, with 300 to 400 cc being the most common range. Implants also differ according to their height and diameter.

Your breast implant size should be proportionate to your body in both size (number of cc’s), projection (height) and width. For example, a 200cc implant may look large enough on a petite woman with a narrow chest, while it would look small on a tall woman with a wider chest area.

What to Expect Before, During & After Breast Augmentation Surgery

In this article he explains what to expect before, during and after breast augmentation surgery. Knowing what to expect helps you make the right breast augmentation decisions to fulfill your goals and meet your appearance expectations.

Before Breast Augmentation Surgery: Preparing

Like most surgical procedures, the preparation process for breast augmentation begins well in advance of the surgery itself. Dr. Ali, a Macomb County board certified plastic surgeon, will go over everything you need to know and do in the weeks prior to the surgery. His patients are advised to stop smoking and using tobacco and nicotine products several weeks prior to your surgery date. Smoking can interfere with the healing process and increases the risks of complications. He may also ask you to stop taking hormonal birth control methods, because they may increase the risk of blood clots.

Around 10 days to two weeks prior to the surgery, you will need to stop taking ibuprofen, aspirin and medications that contain those products, and to stop drinking red wine, because they tend to thin the blood and may increase the chances of bleeding during and after the procedure. Dr. Ali will want to know what vitamin supplements you are taking, and may ask you to stop or reduce your supplement intake for the same reason.

You will need to arrange for someone to drive you home from the surgical center, and if at all possible, Dr. Ali suggests that someone stay with you at least for the first night, to be available in case you need assistance.

You will be given comprehensive instructions on how to prepare for your surgery. It is important that you fully understand your role in the process, partnering with Dr. Ali to ensure a positive outcome.

During Breast Augmentation Surgery: The Procedure

Breast augmentation surgery generally takes less than 2 hours.

Here’s how it works. Your incision will be in one of the following locations: inframammary, which is the crease below the breast; periareolar, the border of the areola and breast skin; or transaxillary, which is through the armpit. Additionally, the implant can be placed either behind the pectoralis major muscle or behind the breast tissue but in front of the muscle. The most popular choice for breast augmentations is submuscular placement via an inframammary approach, meaning most patients prefer their surgeon to go in under the breast.

The inframammary approach begins with an incision in the fold underneath the breast, and the pectoralis muscle is then slit. Next, a pocket behind the muscle is carefully formed and an implant sizer is placed to verify your decision about the size and shape. Then, the sizer is removed, any adjustments are made, the implant is put in place, and the incision is closed. The same procedure is then performed on the opposite breast.

After Breast Augmentation Surgery: Recovery & Downtime

Breast augmentation patients usually stay in a post-operation recovery room for about an hour after the operation, and then are sent home. Many patients have a long-acting anesthetic that works for about 72 hours, leaving them virtually pain free. However, many women report a sensation of chest tightness and soreness. This is perfectly normal.

During the first week and into the second, soreness and swelling are not uncommon. While you’re healing, remember that your breasts will be sensitive to physical contact or jarring movements.

Some bruising can also be expected, although many patients report that they didn’t experience it. There will be scarring, but it should fade over time. However, it likely will not disappear completely.

Dr. Ali usually recommends that his patients wear surgical bras for about two weeks following surgery for added support. You will be asked to make several follow-up appointments to monitor your recovery process. These follow-up appointments are essential, and it is important not to skip them.

Depending on what type of work you do, you may be able to return to work in three to five days. However, there will be activity restrictions that will limit heavy lifting and upper-body exercise for at least two weeks. It is not uncommon for you to feel almost back to normal after one week, but you need to take time to let your body heal completely. You should avoid strenuous activities – anything that could raise your pulse or blood pressure – for at least two weeks.

How Transgender Breast Augmentation Surgery Differs from Cisgender Implants

In many technical aspects, cisgender and transgender breast augmentation (often incorrectly and insensitively referred to as “MTF breast augmentation”) share a number of medical similarities: during both procedures, an implant is placed into the breast which consists of muscle, breast tissue, skin, and nipple. Additionally, the surgical instruments used are essentially the same.

That being said, the characteristics and dimensions of the breast tissue between cisgender and transgender patients are profoundly different. Another common, unfair misconception is that transgender patients typically desire unnatural-looking breasts; when in actuality, the requests of transgender patients are as varied as those of cisgender patients.

In most cases, Dr. Ali’s transgender breast augmentation patients request natural-looking, balanced and proportionate breasts, but Dr. Ali works closely with all of his patients to meet their exact goals when undergoing transgender breast augmentation surgery.

Anatomical Differences in Transgender Breast Augmentation Surgery

Far too often, the anatomic differences between cis and transgender augmentations go unrecognized by both patients and surgeons. Fortunately, Dr. Ali is acutely aware of the 7 anatomical differences between cis and transgender augmentations and takes them into account when caring for all of his patients.

Trans women will usually have a wider ribcage than will a cis woman of the same height and weight. In crafting the perfect breast shape that naturally fits onto the thorax, Dr. Ali considers height, width, volume, and projection in proportion to the transgender breast augmentation patient’s ribcage.

Generally, a trans woman’s nipples will be situated to the outside of the breast mound. This will factor into where the implant is placed, resulting in a nipple that is relatively centered on the breast mound.

In many cases, a trans woman’s nipples will be situated lower on the breast mound. The ideal breast is said to have the breast volume roughly 45% above the plane of the nipple and 55% below. While achieving balance in cisgender women occasionally requires the implant be placed below the natural crease of the breast, it is a far more common occurrence in transgender patients.

All breasts, cis and transgender, are composed of a combination of gland (firm) and fat (soft). Transgender patients typically have firmer and more dense breasts, as their breasts are, for the most part, comprised of glands that, through hormone therapy, have been enlarged. In is imperative for transgender breast augmentation surgeons to account for their patient’s breast tissue malleability, as it affects size of the implant.

The skin of transgender patients is usually thicker and tighter than cisgender patients. As cisgender women’s breast size can fluctuate significantly over the years from weight changes, monthly periods, and pregnancy, their skin can relax and soften over time in a way transgender patients’ often have not been afforded.

In many (but not all) transgender women, the pectoralis major muscle is thicker than in cisgender women. It is almost never thick enough to create any kind of problem, but will require a greater level of attention during surgery, particularly in regard to anesthetic. Over time, this can occasionally result in the muscle pushing the implant slightly out and/or down, but this issue is also faced by cisgender patients, and is not specific to transgender breast augmentation surgery.
