Who is a candidate for blepharoplasty?

In most cases sagging upper eyelid skin is the result of loss of skin tone due to aging. Many people also experience very prominent under eye bags as part of the aging process.

However, “drooping” eyelids can be hereditary. For example, some ethnicities have a flatter facial structure that can leave patients with eyes that they feel do not look sufficiently open.  Drooping eyelids can also, in rare case, be the result of certain medical conditions.

Individuals with loose skin or bulges of fat in their upper eyelid are perfect candidates for upper blepharoplasty. Other good candidates for upper blepharoplasty are people who experience feelings of heaviness in their eyelids, as well as people whose eyelids sag to the point of creating obstruction of their upper field of vision.

What is the best age to have blepharoplasty?

Being a candidate for blepharoplasty has less to do with age and more to do with cosmetic or vision necessity. Anyone at almost any age who is  experiencing sagging, laxity, or bulging of the eyelids can be a candidate for blepharoplasty – as long as they are in reasonably good health.

At AMAE Plastic Surgery in Birmingham, MI some of our blepharoplasty patients have undergone an eyelid lift as young as in their 30s. These younger eyelid lift patients are typically women or men who have eyelids they dislike resulting from heredity or ethnicity. Age related eyelid drooping, however, typically begins in a man or woman’s 40s or 50s. Dr. Ali has treated the greatest number of patients with blepharoplasty surgery in their 50s and 60s. However, even patients in their 70s or older can safely undergo eyelid lift surgery – since it is a safe, routine, and minimally invasive surgery that only requires local anesthetic.

Where are the incisions in blepharoplasty?

During upper blepharoplasty, the incisions are hidden in the eyelids’ upper crease, where they are virtually invisible.  Occasionally, they will extend into the eyelid’s “crow’s feet” crease, but this is typically not necessary.

During lower blepharoplasty, fat is removed by making an incision on the inside of the lower eyelid (the medical name for which is transconjunctival blepharoplasty). In cases of lower blepharoplasty calling for the removal of skin, a small incision is made in the area directly below the eyelashes of the lower eyelid. This also conceals any minor scarring that may occur.

Does blepharoplasty leave scars?

While there is always a scar after any surgery, the eyelid typically heals better than skin on other parts of the body. Eyelid scars are carefully hidden in the crease on the upper eyelid, and along the last line on the lower eyelid – so they are almost undetectable.

In most cases, upper blepharoplasty scars can only be seen – very faintly – when the eyes are closed. When a person’s eyes are open, only someone who knew precisely what they were looking for would be able to notice a blepharoplasty scar. If you are concerned, normal eye makeup typically conceals any blepharoplasty scars on the upper eyelid.

And the position of lower blepharoplasty scars are directly below the eyelashes making them practically invisible.

What’s the difference between blepharoplasty and a brow lift?

Both blepharoplasty and a brow lift are excellent procedures for patients who want to eliminate signs of aging on the upper half of the face. But each procedure is designed to target different concerns. Many patients only need one or the other, while some of our Birmingham, MI patients choose to have both done, for optimal anti-aging!

Anyone with sagging lids whose brow has a desirable arch that is located directly about the eye socket will generally only require an eyelift.

Blepharoplasty for the Upper Eyelids

Blepharoplasty is an outpatient procedure which does not require general anesthesia (being “put to sleep”), and does not need a hospital stay. The eyelid area will be thoroughly numbed, so that patients will feel no pain or discomfort. The procedure itself is relatively straightforward in the hands of a skilled and experienced plastic surgeon like Dr. Ali.

In upper eyelid blepharoplasty, extra skin and fat are removed by way of a small incision made at the natural fold where the eyelid creases. Once the excess tissue has been eliminated, the incision is closed with tine stitches, so that scaring will be concealed in the crease not be clearly visible.

Blepharoplasty for the Lower Eyelids

The process of lower eyelid surgery is very similar.  However, the tiny incision will be made directly behind the patient’s lower eyelid – so no scarring will be visible. Any excess fat and tissue under the skin will be surgically removed, leaving behind no baggy pockets beneath the eyes.

Blepharoplasty Preparation

As with any surgical procedure, it is important to properly prepare to achieve the best results and the fastest recovery. Even though blepharoplasty is an outpatient procedure with minimal risk, results and recovery can be greatly improved by keeping a few things in mind beforehand.

First of all, patients are advised to stop smoking if at all possible. While we understand that this is challenging, the healing process will be much faster and easier if the patient can avoid cigarettes for at least three to four week prior to surgery.

Patients taking blood thinning medications (including aspirin, Ibuprofen, Motrin, Excedrin, Advil, and certain types of herbal supplements) are generally advised to stop taking them for at least one week prior to, and one week following surgery, as these can cause excess bleeding during and after surgery. However, this should be discussed with your prescribing physician. Never stop a medication without your doctor’s approval.

Also, inform Dr. Ali of your medical history. Don’t be shy. Any situation which might affect the procedure or outcome should be discussed during your initial consultation. Dr. Ali will prescribe any medications you may need for pain or infection prevention beforehand, so you can pick them up before surgery.

Finally, make arrangements for someone to drive you home after the procedure is complete, as you will be unable to drive home.

Blepharoplasty (Eyelift) Recovery

Blepharoplasty surgery takes approximately an hour from start to finish for a single eyelid. For those who may choose to do both eyelids at once, this time can be doubled.

It is common to feel some minor discomfort following the procedure. This can last as long as a few days but can typically be treated with over-the-counter pain medication. In some instances, Dr. Ali can prescribe strong pain-relieving medications. This is usually done at the time of the consultation, so that medication may be picked up prior to the surgery.

The presence of any bruising or swelling may last as long as a week before subsiding. A simple cold compress or ice pack should help.

Alternatives to Blepharoplasty Surgery

Every patient is different – and has different desires and beauty goals. An eyelift can be a fantastic solution for regaining a youthful look and removing vision obstructions. But for patients whose cosmetic concerns are wrinkles, rather than sagging lids, an eyebrow lift may be a preferable alternative.

Does blepharoplasty get rid of lines around the eyes?

While browlifts are usually implemented to address hooding of the skin to the sides of the eye, upper blepharoplasty is not performed to treat lines and wrinkles. That being said, when performed in conjunction with other effective treatments such as Botox, upper blepharoplasty patients can achieve such results.

Are blepharoplasty results permanent?

Blepharoplasty patients are able to enjoy permanent results, with most patients never requiring another surgery. In some cases, patients returning to our practice for another procedure may also choose to have their blepharoplasty touched up, but this generally occurs as long as 15 to 20 years after the initial procedure.

Blepharoplasty may also be performed for medical reasons, such as to improve vision in patients who have experienced sagging in their upper eyelids severe enough to obstruct their vision.

Going Home after Blepharoplasty

Blepharoplasty patients are not required to stay overnight at the hospital unless the surgery is being performed in conjunction with an additional procedure, such as a facelift. Blepharoplasty patients must, however, be driven home from their surgery by someone else and should have someone stay with them for the night.

Pain after Blepharoplasty

Any discomfort is typically well managed with over the counter medications such as Tylenol.

Patients who experience intense pain may be prescribed pain medication by Dr. Ali, but such instances are uncommon, with blepharoplasty not being a particularly painful procedure. Any discomfort is typically well managed with over-the-counter medications such as Tylenol.

For the first several days of blepharoplasty recovery, patients should apply cold compresses to the treatment area, which will both reduce swelling and bruising and relieve the patient from pain and discomfort. While cold compresses work wonders to aid in the healing process, blepharoplasty patients should not apply ice directly to the eyelids.

Glasses and Contacts after Blepharoplasty

Not only is it possible for eyeglasses to be worm immediately after the procedure, when patients return the day after their surgery for their follow-up, the frequently arrive wearing sunglasses to mask the swelling caused by the procedure.

While blepharoplasty patients are in the clear to wear glasses almost immediately after treatment, patients should refrain from wearing contact lenses for at least a week after their surgery.

Returning to Work after Blepharoplasty

For blepharoplasty patients who work from home whose daily routine involves non-strenuous tasks such as working on the computer or over the phone, it is possible to return to work the day after their surgery.

But blepharoplasty patients will have stitches in the treatment area for a week after their procedure, and will often experience swelling and bruising around their eyes. For patients who do not mind being seen in public in such a state, it is possible to return to work once three days have passed. For everyone else, it is advised to plan on taking off about a week from work.

Driving after Blepharoplasty

Blepharoplasty patients may resume driving as soon as they are completely off any kind of narcotic pain medication and no longer feels at all restricted by their discomfort in a way that would impede their ability to do so.

Exercise after Blepharoplasty

Most patients report feeling ready to resume their exercise routine about a week after their blepharoplasty surgery. However, in some extremely rare cases, exercise has occasionally resulted in bleeding in some patients up to two weeks after their surgery. For this reason, board-certified plastic surgeon Dr. Ali recommends that all of his patients wait at least three weeks following their blepharoplasty procedure before diving back into their workout routine.

Showers after Blepharoplasty

Unless Dr. Ali has instructed the patient to refrain from doing so in their aftercare instructions, blepharoplasty patients are able to take a shower the night of their upper blepharoplasty surgery.

Postoperative Care after Blepharoplasty

Generally, the incision made during blepharoplasty does not require special care. For the post part, it is alright to get the eyelid wet immediately following surgery, such as by cleaning it with a soft washcloth. Once the sutures have been removed, blepharoplasty patients do not need to apply any kind of special scar ointment to the treatment area, as the healing properties of the eyelid rank among the best in the human body.

4-6 days after blepharoplasty, the sutures will be removed.

While it is exceedingly rare, it is possible for patients to experience bleeding that may apply pressure to the eye. If left untreated, this incredibly uncommon occurrence could theoretically result in blindness. Patients should immediately call their doctor if their eye become swollen after their blepharoplasty surgery, or if they experience severe pain or loss of vision. If they do not immediately receive a response, the individual should go to the emergency room.  But, once again, this is exceeding rare and due to his skill and experience it has not happened to any of Dr. Ali’s patients.
