Why Today’s Breast Augmentation Surgery is Better

Why Today’s Breast Augmentation Surgery is Better

It has been more than fifty years since the first breast augmentation surgery was performed, using silicone implants. Today, breast augmentation surgery is the number one most popular cosmetic surgery in the United States – with 300,000 breast augmentations performed in 2017 alone. This represents a 3% increase over 2016.

One of the reasons breast implant surgery continues to grow in popularity is the improvements made in both the implant materials and the surgical procedures over the last several years.

In this article we share 3 reasons why today’s breast augmentation surgery is better, making it easier than ever before to get the breasts you have always wanted.

While there are lots of different factors that go into getting the beautiful results you want, these three benefits of modern breast augmentation surgery make it

New Breast Augmentation Materials are More Natural

Substantial improvements have been made recently in breast implant technologies – making them both more natural and more durable than ever before.

In 2017, silicone breast implants were used in 87% of all breast augmentation surgeries. While saline implants accounted for 13% of all breast augmentations

The new generation of so-called “gummy bear” breast implants are shaped like a teardrop. The sloped shape – with more fullness at the bottom – is virtually indistinguishable from the contours of a natural breasts.

Made from High-strength cohesive (HSC) silicone gel, these newer implants remain soft and natural-feeling to the touch – just like real breasts.

They even “squish” when pressed against another person – when hugging or being intimate rather than feeling hard like previous breast implants.

New Breast Implant Materials are Sturdier & More Stable

High-strength cohesive (HSC) silicone gel is also breakthrough technology in breast implants because it is the strongest type of silicone on the market. This means that the breast implants far sturdier than they were before, and less prone to breakage or leakage.

When earlier versions of silicone breast implants were damaged the clear gel inside would slowly create leak into the body. Newer silicone implants holds their shape and do not leak if ruptured.

New Breast Augmentation Surgical Techniques are Better

Breast surgeons now have a much better and  more advanced understanding of how to minimize complications from breast augmentation surgery.

With new advances in surgical techniques there is lower risk of infection, capsule contracture, rippling and other complications.

For example, the Keller funnel now allows the surgeon to “fold and slip” the silicone implant into place (kind of like a pastry bag) through a smaller incision. Not only does this procedure minimize scarring, but it also reduces the risk of infection.

How Plastic Surgeons Make Breast Implants Look Natural (Part 2)

How Detroit Plastic Surgeon Dr. Ali
Makes Breast Implants Look Natural (Part 2)

All breast implant surgery alters the size and shape of a woman’s breasts – but it is the skill of the plastic surgeon that results in the breasts looking natural. Women who were dissatissfied with their breast and opted for breast implants almost universally experience greater self-confidence while looking and feeling their best.

Detroit plastic surgeon Dr. M. Azhar Ali has performed more than 2,500 breast augmentations at his Amae Plastic Surgery Center.

Method of Breast Implant Surgery

The way the breast implants are surgically implanted can also affect how natural and attractive that the new breasts look and feel, according to Detroit area plastic surgeon Dr. Ali.

For example, he typically suggests that going under the armpit and placing the implant under the muscle will give the most realistic appearance. However, the type of procedure he uses depends on the woman’s natural breast size and shape, as well as the amount of existing breast tissue.

The size of the implant also matters. For example, larger breast implants cannot be placed by way of the armpit; they must be inserted the through the fold of the breast or areola.

Breast Implant Type

Dr. Ali says that while silicone and saline are both FDA approved, silicone gel implants tend to feel more natural when compared to saline. Silicone gel tends to mimic the natural consistency of breast tissue.

Doctor Ali typically prefers Mentor MemoryShape® Breast Implants for their natural shape, feel, and excellent variety of sizes and silhouettes. These emulate the silhouette of a natural breast, with their teardrop shape. ANd they are filled with a cohesive gel uniquely formulated to retain this natural breast shape.

Realistic Breast Implant Expectations

“During a consultation, I will ask the patient what she expects from breast implants,” says Dr. Ali. “Does she want larger breasts, or to fill them out following childbirth? How pert does she want them to be, and what does she want her cleavage to look like?”

Detroit area plastic surgeon Dr. Ali advises all patients considering breast implants to keep their expectations realistic if they want more natural looking breas implants.

To help with expectations, Dr. Ali recommends that patients bring to their consultation a series of photographs that depict the outcome they expect. Dr. Ali also offers his patients several before and after images that can help his patients visualize what the after effects will look like.

He warns that every woman’s body is different and that a certain size and type of implant may look different on one woman when compared to another. That’s where the doctor’s experience and expertise come into play.

A top plastic surgeon can recommend the best breast implants type, size and method to ensure the patient’s satisfaction. Choosing a highly experienced and qualified board certified surgeon like Doctor Ali is the best way to ensure natural looking and feeling breast implants.

History of Silicone Breast Implant Regulation (Part 1)

History of Silicone Breast Implant Regulation (Part 1)

Most board certified plastic surgeons, like Detroit’s Dr. Ali, now prefer silicone breast implants for their patients because of their natural look and feel, and excellent safety record.

In this two part article Detroit area breast implant surgeon Dr. Ali discusses the regulations that have affected silicone breast implants over the years – ensuring patient safety.

Silicone Breast Implant Regulation: 1960s

Gel-filled silicone breast implants were introduced to the U.S. in 1962, and in common use by 1964.

This new technology was imperfect at first. In large part this was due to lack of study or regulation on this new technology.

But thanks to advancements in technology, rigorous scientific testing, and intensive regulation by the FDA, silicone breast implants are now very safe.

Silicone Breast Implant Regulation: 1970s

Before 1976 breast implants were not highly regulated. In 1976 the U.S. Congress passed the Medical Device Amendments to the Federal Food, Drug, and Cosmetic Act. Under this law, breast implants were considered “Class II” (moderate risk) medical devices – and as such they had to comply with general controls and performance standards.

At that time the FDA reviewed new breast implants through a process called the 510(k) Premarket Notification process, like any other Class II products.

Silicone Breast Implant Regulation: 1980s

In the early 1980s, the FDA began to identify certain local complications and adverse outcomes of these older silicone breast implants. Some reports even described cancer and connective tissue disease in a small number of women with breast implants.

As concerns arose about the safety of silicone breast implants rose, the FDA reclassified breast implant into “Class III” (higher-risk), which requires premarket approval. Breast implant manufacturers now had to provide data demonstrating the breast implants were safe and effective BEFORE they could be sold.

Silicone Breast Implant Regulation: 1990s

In 1992, the FDA still had concerns that the breast implant manufacturers were not adequately addressing public concerns about certain complications. At that time a higher incidence of implant rupture and silicone leakage was being seen.

So, the FDA removed all silicone breast implants from sale in the US, and required all manufacturers to submit premarket approval applications that contained detailed data on safety and effectiveness.

However, in the meantime, the FDA did allow manufacturers to provide silicone breast implants for reconstruction after mastectomy, correction of congenital deformities, or replacement of existing implants. So, the manufacturers enrolled women who received silicone breast implants for medical (not cosmetic) purposes in performance and safety studies.

Then in 1999, the Institute of Medicine (IOM) released a detailed report compiling the results of all of these published and ongoing studies on breast implants, called Safety of Silicone Breast Implants.

This report concluded that there was no evidence that silicone breast implants caused systemic health effects such as cancer or autoimmune disease. And while it did identify rare local complications (which included rupture, pain, capsular contracture, disfigurement, infection, and repeat surgeries) silicone breast implants were now recognized as generally safe.

Silicone Breast Implant Regulation: 2000s

After years of studies and technological improvements, the FDA approved silicone gel-filled breast implants in November 2006.

As with any surgery, there is always a small number of possible local complications and adverse outcomes. However, the FDA concluded that the benefits and risks of breast implants were sufficiently well understood for women to make informed decisions about their use.

Initially, in 2006, the FDA only approved Mentor’s MemoryGel Silicone Gel-Filled Breast Implants and Allergan’s∗ Natrelle Silicone Gel-Filled Breast Implants – followed six years later by Sientra’s Silicone Gel Breast Implants.

Silicone Breast Implant Regulation: 2010s

Advanced technologies in breast implant look, feel and safety continued to be developed. And in 2013 Mentor’s MemoryShape and Allergan Natrelle 410 Highly Cohesive Breast Implants were approved.

Today these five breast implants remain the only 5 breast implants that are FDA approved for use in the US.

Pregnancy, Breast Feeding and Breast Implants

Pregnancy, Breast Feeding and Breast Implants

Detroit area board certified plastic surgeon Dr. Ali has performed thousands of breast implant surgeries on women before and after pregnancy. Many women wonder the effect of breast implants, if any, on pregnancy and breastfeeding. about In this article Doctor Ali answers your questions about breast implants and pregnancy or breastfeeding.

No Breast Implants for Women who are Pregnant or Nursing

While it seems like “common sense” it is important to note that breast implant surgery should not be performed on a woman who is pregnant or nursing. However, women may get breast augmentation surgery before or after becoming pregnant and/or nursing.

Always ensure you are not pregnant before undergoing breast implants, taking a pregnancy test if there is any doubt or possibility that you may be carrying a child.

Breast Implants Before Becoming Pregnant

Some patients have asked us if having silicone gel breast implants could harm babies whose mothers became pregnant after getting breast augmentation.

We understand that nothing is more important to you than the health of your children! There is no evidence to shows that silicone breast implants have any harmful effects on children born to women with implants.

Two clinical studies in humans have found that there is NO increase in the risk of birth defects in children conceived and born after a women has had breast implant surgery.

Breast Implants Effect on Breast Feeding

In some women, there is a very small possibility that breast implant surgery might interfere with the ability to breastfeed. A very small percentage of women who have had breast augmentation surgery produce less milk, or are note able to produce milk at all. Some women with breast implants have also reported that breastfeeding is somewhat more painful after having breast implants.

In a study conducted on MemoryShape™ Breast Implants (the type of implant Dr. Ali uses), only four of the 44 breast augmentation patients who attempted to breast feed following breast implant surgery experienced difficulty. This means that 91% of the women in the study who attempted to breast feed after receiving breast implants had no difficulty.

The type of surgical technique used seems to have some effect on the likelihood of breastfeeding difficulties. For example, peri-areolar surgical approach (in which the surgeon uses an incision around the areola/nipple) has a slightly higher incidence of breastfeeding difficulties than other techniques.

Studies have also measured silicon levels in breast milk. Those studies did not indicate higher levels of silicon in breast milk from women with silicone gel breast implants when compared to women who did not have implants.

In fact, The Institute of Medicine (IOM) and The American College of Obstetricians and Gynecologists (ACOG) both have issued statements encouraging women with breast implants to breastfeed, stating that “Breastfeeding should be encouraged in all mothers when possible, including those with silicone breast implants.”

Decision Checklist for Getting Breast Implants

Breast Implants: Decision Checklist –

The surgical procedure for breast implants consists of choices you and your plastic surgeon need to make to ensure the best possible outcome and the highest degree of satisfaction with your new breasts.

In this article, Detroit area board certified plastic surgeon Doctor Ali has put together a “checklist” of the decisions you will need to make when deciding upon breast implantsurgery.

☐ Plastic Surgeon

Your surgical team – including your plastic surgeon, nurses, and anesthetist – is the most important decision you need to make.

Make sure to inquire about the number of breast implant surgeries your plastic surgeon has performed, as well as whether he or she is board certified.

Detroit area plastic surgeon Doctor Ali is double board certified, and has performed thousands of breast implants. In addition to being voted one of Hour Detroit’s “Top Docs” – many doctors seek out Doctor Ali when they need plastic surgery for themselves and their families.

☐ Type of Breast Implant

Silicone breast implants are filled with silicone gel, which feels remarkably like natural breast tissue. Saline breast implants are filled with sterile salt water. While they do provide a uniform shape, firmness and feel, they are less popular than silicone implants because they do not feel as “natural”.

☐ Size and Shape of Implant

In addition to the size that will look best on your body and achieve your appearance goals, the profile, or projection (height of the top of the breast implant from your chest) must be chosen.

Learn more about choosing your breast implant size HERE.

Learn more about choosing your breast implant shape HERE.

☐ The location of the incisions

The location of the incisions made to insert the breast implants result in different recovery times, as well as varying amounts of scarring.  There are 3 standard types of breast implant incisions:

1: Transaxillary Breast Implant Incision, in which an incision is made through a canal placed in the natural fold of the armpit tissue;

2: Periareolar Breast Implant Incision, which involves making the incision around the circumference of the areola (the pink or brown skin around the nipple);

3: Inframammary Breast Implant Incision, made underneath the breast, in the fold where the breast and skin attach. – is not recommended for silicone gel implants such as MemoryShape.

☐ Where the implants will be placed in your breasts

Breast implants may be placed either:

1:  Subglandular – putting the breast implant directly behind the mammary gland, but in front of the muscle; or

2: Submuscular – breast implant is positioned completely behind the chest muscle wall.

Submuscular vs Subglandular Breast Implants

Pros and Cons of Submuscular vs Subglandular Breast Implants –

In addition to choosing the size, shape, material, size and “projection” of your breast implants – the placement of your implant must be considered. Breast implants can be placed over or under (or in some cases between) the muscles in your chest.

Subglandular breast implants put the breast implant directly behind the mammary gland, but in front of the muscle.  Submuscular breast implants are positioned completely behind the chest muscle wall.

Choosing the right breast implant placement is an important decision made by you and your breast augmentation surgeon. Many factors come into play in making this decision, including: the type of implant; the increase in size; the amount of available breast tissue; your skin and muscle tone; your size; lifestyle; and your appearance goals.

Detroit are plastic surgeon Dr. Ali has helped thousands of women achieve the look and feel they desire with perfectly chosen and expertly “placed” breast implants. In this article we explain the pros and cons of Submuscular vs Subglandular breast implant placement.

Caring for Your Breast Implants

Breast Implants: Mammograms & Long Term Care –

Just as there are some things you should do to make sure your breasts stay healthy, there are also steps you should follow to care of your breast implants over time. Mammograms, breast exams, protecting your implants from certain types of damage, and regularly monitoring for breast implants for rupture are all very important.

Mammograms and Breast Implants

Whether or not a woman has breast implants, having a mammogram is the best way to detect breast cancer. However, there are some unique mammogram considerations for women with breast implants.

Breast implants can make it harder for the technician to perform a mammogram. Breast implants can also make it harder to identify breast cancer on a mammogram.

As every woman knows, the mammogram machine squeezes the breast to make it flat while taking a picture. The pressure from this squeezing could make your implant rupture or cause gel bleed.

Be sure to tell the mammogram technologist that you have breast implants before the procedure, so she can use special techniques to get the best possible views of your breast tissue, taking steps to ensure that your breast implants will not rupture due to the mammogram. Show your Device Identification Card (that you will receive after surgery) to the mammography technician.

Breast surgeons recommend having a mammogram before your breast implant surgery to establish a “baseline” for comparison in the future. Then women are encouraged to have another mammogram 6 months to 1 year after your implant surgery to establish another ”baseline” with the breast implant present.

After that women with breast implants should have a mammogram every 1 to 2 years, or as advised by their doctor, just as women without breast implants should do.

Other Breast Exams with Breast Implants

You should also perform self-breast exams once a month. After your period ends, is a good time to examine your breasts. Your doctor can show you how to do a self-breast exam, and teach you to tell the difference between your breast implant and breast tissue. If you see or feel that something has changed, talk to your doctor promptly.

It can sometimes be hard for you to feel changes in your breast when the implant is there, so it is important to have regular breast exams by a doctor, as well.

Preventing Breast Implant Rupture

Breast implant are quite durable, and are built to handle day to day bumps, hugs, and other everyday activities. Most surgeons indicate that you can even continue to participate in sports without restrictions. So it is not necessary to be overly concerned about rupture. However, physical trauma (e.g. an auto accident or fall), excessive repetitive wear and tear, can lead to a rupture in some cases.

A medical condition called capsular contracture can also cause rupture in a very small percentage of breast implants. Massaging your breasts regularly can help keep the surrounding soft and malleable, and prevent capsular contracture. Always alert your plastic surgeon if you notice the formation of any hardened scar tissue so that he can intervene before a rupture occurs.

If you or your doctor suspect a rupture or “gel bleed” of your breast implants, an MRI should be taken as soon as possible. An MRI is the best way to detect breast implant ruptures or leaks.
