Butt Augmentation: Brazilian Butt Lift, Implants, Butt Lift Surgery or Sculptra?
A full, round, shapely buttocks has long been idealized in beauty and art. But not every women is born with a lovely behind. And, even when women have been blessed with the perfect bottom in their youth, age, weight fluctuation, inactivity and loss of muscle tone can result in drooping, sagging and flattening of the buttocks over the years.
Many women come to AMAE in the Bloomfield Hills area seeking a rounder, fuller or more shapely buttocks – but are not quite sure of their options. Surgical butt implants; (3.) Butt Lift Surgery; and, (4.) Buttocks fillers such as Sculptra.

Brazilian Butt Lift
A Brazilian Butt Lift (“BBL”) is a procedure that transfers a patient’s own fat cells from unwanted areas of the body (usually from the abdomen, “love handles,” back or thigh) to their bottom. A skilled and experienced plastic surgeon like Dr. Ali can effectively “sculpt” the body by removing fat using liposuctions – and then re-injecting that fat into the buttocks to plump and round it out.
Because the body’s own fat is used to “fill” the buttocks, the procedure is 100% natural. And because entire, living fat cells are transferred into the buttocks, a Brazilian Butt Lift lasts for years.
The biggest advantages to the Brazilian Butt Lift are the natural feel and longevity. Patients typically have as much fullness in their behind 10 years after surgery as they have at the 6 month mark. And because their own fat is used, the feel is exceptionally natural. Additionally, Brazilian Butt Lift uses liposuction to remove fat from the abdomen, hips, waist, back and thighs. So this procedure plumps your backside while trimming down unwanted body fat in other areas!
The downside to a “BBL” is that it can result in infection, blood clots and other dangerous side effects when not performed by a skilled and experienced plastic surgeon. This is because the buttocks contains several very large blood vessels which, if accidentally injected with fat, can result in that fat traveling to the lungs and causing deadly pulmonary embolisms.
A Butt Implant is a procedure that uses a silicone implant that is surgically inserted into the butt to give it a fuller and more rounded look. Buttock implants are more limited because they only deliver volume to one specific area of the behind – and cannot be sculpted and contoured like a Brazilian Butt Lift to custom shape the buttocks.
Buttock implants do offer the advantage of being able to add substantial volume, without needing to harvest fat from other areas of the patient’s body. So they can be a suitable option for thinner women who desire more volume in the buttocks.
However, buttocks implants tend to feel somewhat less natural than fat grafting (BBL). Additionally, because a buttock implant requires surgery and an incision there is more potential for complications such as infection, capsular contracture (scar tissue formation), and wound healing problems at the incision site. Butt implants can also become displaced or cause, sagging over time.
For these reasons, butt implants are generally not preferred by most board certified plastic surgeons for use in buttock augmentation.
Butt Lift Surgery
Many board certified plastic surgeons prefer butt lift surgery – without implants or fat augmentation. Butt lift surgery (gluteal lift) improves the shape and tone of the underlying tissue that supports the skin and fat in the buttock area without the need for implants of fat grafting.
Aging, pregnancy, weight gain or loss, hereditary factors, and even hormonal decline causing muscle loss can contribute to the poor tissue elasticity that results in sagging of the “butt cheeks”.
During butt lift surgery, excess sagging or flabby skin is removed – and a higher, smoother and more pleasing contour is created. And if fat removal is also required, liposuction can be added during the procedure for better contouring.
The disadvantage to a traditional gluteal lift is that it will not make the butt larger. But it can make it higher and rounder as well as improve a dimpled, irregular skin surface. And it offers the advantage of being less risky than a BBL, and more natural looking and feeling than implants.
Sculptra is a filler that is injected into the buttocks that immediately adds volume and also triggers the body’s production of new collagen over time – for natural, long-lasting results.
Sculptra requires no incisions and has been used on over 300,000 patients with excellent results. Because Sculptra is a “biocompatible” substance there are also no known allergic reactions to Sculptra. The biggest advantage to Sculptra is that there is virtually no downtime after treatment. So patients are back to their daily routine immediately following treatment.
The biggest downside to Sculptra is that the results are very subtle – and it will not achieve the more dramatic results of surgical implants or BBL fat grafting. Also, unlike butt lift surgery – which is “one and done” – Sculptra may require a few sessions to get spaced every couple months to achieve sufficient volume.
